DRILL SCENARIO 08/11/82 OBJECTIVES FOR ANNUAL EXERCISE I.

NOTIFICATION AND COMMUNICATION A.

To test and evaluate the adequacy of the TMI notification alert capabilities.

B.

To test and evaluate the ability of key TMI Emergency Response Personnel to implement notification procedures including initial and continuing notification, coordination and updating of information.

C.

To test and evaluate the ability of TMI Emergency Response Personnel to accurately and efficiently assess, classify and declare an abnomal or accident situation and disseminate vital infomation to appropriate agencies.

D.

To test and evaluate the THI Emergency Communications Systems between licensee's onsite and offsite facilities, and between licensee's facilities and outside agencies (i.e. state and local facilities).

II.

DIRECTION AND CONTROL A.

To test and evaluate the TMI Operations Staff's ability to determine the cause of the accident, terminate or limit radioactive releases, and bring the plant to a safe shutdown condition.

B.

To test and evaluate the adequacy of the TMI Unit 1 and 2 Emergency Plans and respective Implementing Procedures in tems of management and control of an emergency situation involving the entire station.

C.

To test and evaluate the licensee's capability to account for personnel inside the Protected Area and provisions to evacuate site personnel.

D.

To test and evaluate the Itcensee's capability to activate and staff all required emergency response facilities and to properly manage a station event.

E.

To ensure provisions exist for establishing and sustaining the mitigation process on a long tem basis (recovery phase, emergency staffing, etc.).

DRILL SCENARIO 08/11/82 III. ACCIDENT ASSESSMENT A.

To test and evaluate the licensee's ability to deploy radiation monitoring teams, communicate with them, and assess information received.

B.

To test and evaluate the licensee's capability to assess and determine the extent of an emergency, and make appropriate recommendations concerning protective actions.

RCS leak rate determination should begin.

T = 15 Hi Standpipe Level alarm on RCP 1A No. 2 seal (computer (0745) only ).

T = 40 RC Drain Tank high level alarm (due to RCP-1A seal).

T = 60 RML-1 Hi and Low Channels Spike, resulting in a HIGH alarm then (0830) returns to normal.

Note:

RCS sample should be requested.

T = 65 RCS leak rate calculation shows 1 gpm.

UNUSUAL EVENT T = 90 CAV-2's packing fails, while the RCS sample is in progress, which (0 900) releases airborne radioactivity to the Auxiliary Building.

T = 91 RMG-11 Alert alarm.

RMA-8 Gaseous and Radioiodine Channels increasing.

T = 95 RMG-11 High Alarm (0905)

T = 100 RCP-1A seal fails completely -- RCS leakrate increases to 350 GPM.

High Make-up Flow Alarm (F-1-8160 GPM).

RC Drain Tank Rupture Disc ruptures allowing pressure to increase in Reactor Building. Reactor shutdown should commence.

NOTE:

1. Dispatcher informs the Shift Supervisor:

"We will definitely have a loss of offsite power if you exceed 2 percent / min ramp down".

2. MU-Y-217 open with MU-P1A and (10-P-1B running l

DRILL SCENARIO 08/11/82 T = 105 Maintenance workers report to ECC the outer access door will not (0915) open.

NOTE:

R.B. pressure has increased and is holding the outer door shut. A purge must be started to equalize pressure so the workers can operate the airlock.

If communication is attempted with the inaintenance workers in the RB, the OSC observer will inform the OSC that they are okay but feeling faint. Workers try energency door - but it's jammed shut.

T = 110 R.B. Purge is commenced (Exhaust valves slam open).

Smoke is (0920) coming from 1A-ES Valves Motor Control Center (cannot energize because of feeder breaker damage). Offsite fire fighting assistance not required.

T = 112 OSC observer acting as one of the maintenance workers in the RB, (0922) infonns the OSC that the other worker has fainted, is bleeding and unconscious.

SITE EMERGENCY NOTE:

Accountability should commence.

T = 130 Reactor is shutdown.

MU-V36 fails as is.

MU-V14A fails shut.

MU-V16A and B fail shut.

BS-V1A fail shut.

BS-V2A fail shut.

BS-V3A fail shut.

Note:

1. If operator attempts to cross-connect MU-P-1A, MU-V688 cannot be opened due to galled shaft.
2. Accountability indicates two (2) people are unaccounted for in the protected area and Search and Rescue should be ini tiated.

T = 140 MU-P-1B trips on low oil pressure.

Pressurizer Heaters are -lost due (0950) to low pressurizer level.

Incore thermocouples - 595'F.

NOTE:

1. The cause of the low oil pressure was no oil in the reservoir and the pump motor has seized.

DRILL SCENARIO 08/11/82 T = 150 R.B. pressure is equalized allowing the injured contaminated (1000) worker's removal - (RMC scenario)

NOTE:

Purge cannot be secured because exhaust valves are inoperable, (i.e. Motor burned up and air operated is jammed by opening with pressure in buildi.ng).

RCS voiding starts to occur.

Incore thermocouples -- 632*F.

T = 170 Site Evacuation should be ordered.

NOTE:

1. Fif teen (15) volunteers will.actually be evacuated to the Middletown Junction Substation.
2. Monitoring team at Middletown Junction Substation find five (5) persons contaminated.

(These persons should be dressed in PC's and taken to the AEOF (Crawford Station) for decontamination.)

3. Contaminated persons arrive at the AE0F for decontamination (begin decontamination process).

T = 175 Auto wreck near Unit 2 Administration Building requiring Unit 2 (1025) response with injured personnel and a fire potential requiring offsite fire and ambulance support.

T = 195 RCS pressure starts increasing.

RCS leakage starts increasing due to increased pressure.

T = 200 RCS Code Safeties lif t continuously, increasing coolant (1050) loss to 700 gpm.

2 T = 208 Incore Thermocouples --- 705 F.
