Dear Mr. Keppler:

Deficiency 81-04 10CFR50.55(e)

Certifications of HVAC Materials On October 2, 1981, Illinois Power verbally notified Mr. H.M.

Wescott, NRC Region III, of a potential reportable deficiency per 10CFR50.55(e) concerning inconsistencies in the certification for Clinton Project HVAC materials installed by the Zack Company. This initial notification was followed three interim letters: L.J. Koch letter U-0325 to J.G. Keppler dated November 4,1981, L.J. Koch letter U-0413 to J.G. Keppler dated February 3, 1982, and W.C.

Gerstner letter U-0474 to J.G. Keppler dated May 3, 1982. Our investigation of this matter is still in progress, and this serves as an interim report per 10CFR50.55(e)(3).

e Zack Company has provided Baldwin Associates (Illinois Power Contractor) a final list of nonconforming purchase order /CMTR documentation packages that.were identified during Zack Company's investigation into this matter. A total of fifty-three (53) nonconformances were identified, with status as follows:

1. Twenty-one (21) nonconformances require Baldwin Associates /Sargent and Lundy (Architect Engineer) review and disposition. This review is presently in-process.
2. Thirty-two (32) nonconformances have been investigated and closed by the Zack Company.

In addition to the actions being taken to resolve the iden-tified documentation discrepancies, Illinois Power Quality Assur-ance has conducted an evaluation / verification of Zack Co. correc-tive action on this matter. This evaluation / verification revealed problems in the Zack Co. closure of a number of discrepant documen-tation packages, and with other Zack Co. corrective action measures being taken to correct deficiencies and to prevent further recurrence. Until these problems have been adequately resolved, and Sargent and Lundy has completed their review of the twenty-one (21) discrepant packages, an evaluation of this matter per 10CFR50.55 (e) cannot be completed.
