(NRC INSPECTION REPORT NOS. 50-327/90-25 AND 50-328/90-25)
This refers to the- Special Maintenance Team Inspection led by B. R. Crowley
of this office on July 23-27', August 6-10, 20-24 and September 14, 1990. The I
. inspection included a' revi'ew: of activities authorized for your Sequoyah
- facility. At' the conclusion of the inspection, the findings were discussed .
with those members of your staff identified in the report. l
. Areas examined during the inspection are identified in the report. Within
these areas,. the inspection consisted of selective examinations of procedures
and ' representative records, : interviews with personnel, and observation of
activities in' progress.
The- inspection findings are summarized in the Executive -Summary of the
report. -In general, the findings show that you have developed a good .
maintenance program with satisfactory implementation. Particularly noteworthy

y'- in ' contributing t'o an overall good program was a strong maintenance management l
zwith support _ from - plant = management and a strong corporate maintenance . l
organization involved in site maintenance activities. Commitment to a strong
maintenance program was evident. Your attention is invited to the weaknesses
identified in the report. . In particular, weaknesses in the area of cleanliness
and material condition -of' low-voltage vital equipment should receive attention..
-Based on the results of this inspection, certain of your activities appearedito

be in violation of NRC: requirements, as specified in the enclosed Notice of
- Violation ' Notice). We are. concerned about the violation (failure to control'
~
cleanliness in _ low-voltageL vital equipment) because the conditions in the
instrumentation Mand control cabinets could have rendered the. equipment
. inoperable .in a seismic : event. In addition, a similar violation was
. identified on high-voltage. (480 volt and 6900 volt) equipment in early 1988.
(See ' Inspection Report- No. . 50-327, 328/88-06). It appears that corrective
action for the previous violation was ineffective. P

specified in the encicsed Notice when preparing your response, in your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence. After reviewing your response to this
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements,
in accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of
this letter and its enclosure will be placed in the NRC Public Document Room.
The responses directed by this letter and the enclosed Notice are not subject to
the clearance procedures of the Office of Management and Budget as required by
the Paperwork Reduction Act of 1980, Pub. L. No.96-511.
