Appendix A NOTICE OF VIOLATION Detroit Edison Company Docket No. 50-341 Based on the inspection conducted en September 27, 28 and October 11 -
13, 1978, it appears that certain c f your activities were in noncom-pliance with NRC requirements, as ncted below. The item is an infraction.
10 CFR 50, Appendix B, Criterion IVI, states, in part, that " Measures shall be established to assure that conditions adverse to quality, such as....... deviations,
. are promptly identified and corrected."
Enrico Fermi Unit 2 QA Manual, QA Procedure No.17. Rev. 4, Paragraph 17.1.1, states, in part, that " Quality Assurance and Quality Control personnel shall promptly identify and report on conditions adverse to quality, such as.
.. nen;onformances,.... and procedural nonconformances.
Contrary to the above, on Septed er 27, 1978, the inspector determined that Wismer and Becker Contracti g Engineers (W&B) personnel failed to promptly identify, report anc take corrective action relative to the removal of defects on well No. E21-3147-0W2 on drawing No. 6M721-3147-1. The inspector noted that an area over 3/16" wide and approximately 1 " long in the root pass of the above weld had been ground through ard as a result the weld end configu-ration no longer met the requirements of W&B welding procedure No. 103, Rev. 9 Upon talking with the welder, it was the inspector's understanding that welding on the above weld joint was ready to resume.

The failure to promptly ider:ify, report and take corrective action is a repetitive item that w;s identified previously in IE Reports No. 50-341/78-08 and No. 50 241/78-11.
