A Fitness-For-Duty (FFD) inspection is scheduled to be conducted at Quad Cities Nuclear Power Station the week of January 22-25. 1991.
Mr.' J. R. Kniceley of this office h6s been desi nated as the primary contact for this inspection and can be contacted at tele hone number (708) 790-5641.
As part of the inspection process, FFD written policies and procedures will be reviewed in the NRC Region 111 office prior to the onsite inspection.
You are l
requested to provide us a copy of the procedures and policies pertaining to your FFD program.
These documents will be returned to you or destroyed if requested after i.he inspection report has been prepared. Any of the documents that are considered as exempt from public disclosure in accordance with 10 CFR 2.790 should be identified as such and a statement of the reasons for nondisclosure should + iso be proiided as required by 10 CFR 2.790(b).
The FFD policies and procedures shoult' be provided to us by January 7,1991.
i FFD training material will be reviewed onsite and does not have to be sent to us.
During the inspection, interviews will be conducted with the onsite FFD manager, the Medical Review Officer, the Erployee Assistance Program manager, and personnel responsible for the ansite collection and, if appropriate, onsite testing facility.
If fu sible, please contact Mr. J. R. Kniceley by January 14, 1991, to arrange an interview schedule fw the above personnel.
Interviews of randomly selected licensee employees and contractors will also be conducted during the'onsite inspection. A room to conduct the interviews will be required.
FFD program audits and program records (including training vecords, personne1' records for FFD program personnel, records of pre-access at thoriz6tian inquiries, executed contracts with contractors / venders, and records of chemical test results and appeals, etc) should be available for review.
If any of these records are located offsite, please inform Mr. J. R. Kniceley as soon as practical.
