This refers to the inspection. conducted by Mr. R. C. Barr, Mr. J. F. Melfi and-

Mr. B. J. Olson of this office on September 2, 1990 through October 6, 1990 of

activities authorized by NRC License No. NPF-1, and to the discussion of our

findings held by Mr. Barr and Mr. Melfi with Mr.' Robinson and other members of

your staff periodically during the inspection.

Areas examined during this insaection are. described in the enclosed inspection

report. Within these areas, tie inspection consisted of selective

,

examinations of procedures and representative records, interviews with

personnel and observations by the inspectors.

This inspection identified two Surveillance-Program weaknesses. .First, some

Trojan Surveillance procedures were found which did not always establish

appropriate acceptance criteria.

Second,' administrative controls do not

.

appear to provide a requirements that equipment conditional surveillances are

i

,

performed.

These deficiencies appear to have caused previous violations and

,

i

require your close attention to ensure these weaknesses tre resolved.

At the

October 15, 1990 exit, you committed, by November 1,1990, to evaluate a

long-term program for procedural upgrada of the technical content of

,'

L

surveillance procedures.

The Trojan Surveillance Program should be improved

in the near term so that PGE has full confidence that acceptance criteria are

)

accurate and that conditional surveillances are identified when required.

presentation by your staff on the Trojan Surveillanca Program improvements

'

should be planned for our next management meeting.

The one level V violation was issued to direct management attention to the

inservice testing (IST) program' commitments and requirements.

Although of

minor safety significance, some IST issues discussed in our report were

identified ir the 1985-1987 time frame and are still open.

Previous

correct.ive actions do not appear to have been fully effective. . Consequently,

we request that your response to our Notice of Violation also address how the

.IST related Design Change Packages and Requests for Evaluations are being

tracked and scheduled to ensure completion.

Based on the results of this inspection, it appears that certain of your

activities were not conducted in full compliance with NRC regulations, as set
