Based on the results of the NRC inspection conducted on October 12-13, 1978, it appears that two of the activities at your TRIGA MARK F reactor facility were not conducted in full compliance with the conditions of the NRC Facility License No. R-90 as indicated below:
1.
Contrary to the requirements of the TRIGA MARK F Technical Specifi-cations, Paragraph E-4, the periodic maintenance of the transient (pulse) rod driv.e cylinder was not performed within the required eight-month interval. The last documented maintenance was performed on January 12, 1978.
l This is an infraction.
2.
Contrary to the requirements of 10 CFR 50, Paragraph 50.54(1-1),
the following activities were not conducted as required by the referenced paragraphs of the NRC approved Operator Requalification Training Program:
a.
The periodic and systematic performance evaluations of licensed individuals by supervisory personnel had not be performed and documented as required by Paragraph VII.
b.
The abnormal / emergency procedure review with licensed in-dividuals required hy Paragraph V had not been documented as required by Paragraph VIII.
