Inspection on Sectember 13 and 14,1978 (Recort No. 50-247/78-30) .

Areas Inspected: Routine, announced inspection of the licensee's response '

to IE Circular 78-08 (dated May 31,1978) entitled " Environmental Qualif-

ication of Safety Related Electrical Equipment at Nuclear Power Plants."

This inspection involved eight inspector-hours at the corporate office by

one NRC regional based inspector.

Results: No items of noncompliance or deviations were identified.

2. Inspection Purcose

A management interview was held at the start of the inspection on

September 13, 1978, to explain that the purpose of the inspeEt' ion

was to review the actions taken by the licensee in regard to Inspec-

'

tion and Enforcement Circular 78-08, transmitted to the licensee

on May 31, 1978.

3. Insoection Scoce

Thc areas reviewed during this inspection encompassed those items

listed in IEC 78-08, involving problems that have been identified

in ensuring environmental qualification of safety-related electrical

components. The following were addressed:

a. whether a specific licensee individual has been assigned

responsibility for reviewing identified problems and docu-

mentation referenced in IEC 78-08;

b. whether a comparison has been made between the plant specific

circumstances at Indian Point, Unit 2 and problems identified

by IEC 78-08;

c. what actions have been taken by the licensee in regard to the

subject reviews, the status of any corrective actions planned

or taken, and the schedule for finishing the -review of any

areas not yet complete;

d. a sampling review of documentation which supports the environ-

mental qualification of electrical components, including

' electrical connectors, containment electrical penetrations,

l terminal blocks, cables and cable splices, transmitters and

valves.

Except as noted in Detail 5, no inadequacies were identified during '

the sampli.ng review of the above items. j

5. Findings

The following unresolved items were identified during the inspection l

of the licensee review of IEC 78-08 and require further action by l

the licensee.  !

a. Cable Solices

1:

The licensee has~ established that during construction, splices

in safety-related components and penetrations were to be accom-

plished to the " Splicing and Terminating Instructions," Draw-

ing 9321-LL 3011 which specifies the type of seal to be in- l

stalled. Environmental qualification of this type of seal +

and an in-plant verification of installed splices has not been

'

b. Stem-Mounted Limit Switches

The licensee identified in a response to IE Bulletin 78-04,

(March 23,1978), the future replacement of a number of limit

switches. A modified operating procedure is currently in ,

effect to monitor limit switch positions. (247/78-30-04)

c. Motor Ocerated Valves

The licensee is reviewing the WCAP 7410-L for verification

of environmental qualification of safety-related motor oper-

ated valves. (247/78-30-02)

d. Terminal Blocks

The licensee has not identified documentation for the environ- ,

mental qualification of terminal blocks with the associated

terminal boxes and cable connections for safety-related

equipment. (247/78-30-03)

The above unresolved items require the following:

- Correlatior of actual installed material vs. documented generic

test data.

- Review of the quality assurance records for the environmental

tests.

- Further review by the licensee's engineering staff as to the

adequacy of the environmental testing for the existing equip-

ment installed in safety-related systems.

The licensee expects to complete the review by October 30, 1978.

This review will include visual verification of the existing equip-

ment on a sampling basis.

These items will be reviewed ca a subsequent inspection.

6. Unresolved Items

,

Unresolved items are those items for which more information is

required to determine if the items are Acceptable or Items of

'

Noncompliance. Unresolved items are contained in Detail 5 of

this report.

7. Exit Interview

A management meeting was held with licensee personnel (denoted in

Paragraph 1) at the conclusion of the inspection on September 14,

1978. The purpose, scope and findings of the inspection were

discussed as they appear in this report. ,
