Dear Mr. Seidle:

This letter is in response to the inspection of Messrs. J.P. Jaudcn and M.E. Murphy during the period of May 10-12, 1982 and the notice of violation dated June 11, 1982 for the University of Utah TRIGA Reactor license R-126.

Items A&B - Although no signature was present for those dates, the initials of the senior operatior performing the operation were included as part of the last entry. The operators have been cautioned to include their signature for all operations entered in the log book.

Item C - According to technical specification 6-3, " Written procedures reviewed and approved by the Reactor Safety Ccmnittee'l shall be in effect and followed for the follcving itcnns. . . . "a. Startup, operation, and shut down of the Reactor, b. Installation and renoval of fuel elenents..".

All these procedures have been reviewed and approved by the RSC and they were being followed at the time of the inspection. The inspectors felt that the forms for reactor startup listed in appendix B of the technical specifications were not part of the operations log books. We subnit, that these fonns are part of the reactor operation log books and meet the criteria of T.S. paragraph 4.5. The last part of the Critical Approach Checklist is to make entry in the reactor log book which contain reactor operations other than startup and shutdcwn procedures. Annotations were made on these forms when no reactor operation was performed. To alleviate this problem forms B1-B3 have been revised so that enteries are made in the operations log at the beginning of checkout procedure B.l. and the necessary annotations are made in the Jog book if the reactor is not operated.
