Dear Mr. Keppler:
10 CFR Part 21 Defect PGCC Termination Modules Docket No. 50-461/CPPR-137 On December 7, 1981, Mr. A.J. Budnick of Illinois Power notified Mr. H.M. Wescott, USNRC Region III, of a potential reportable defect per 10 CFR Part 21 regarding PGCC Termination Modules supplied by General Electric Company.
Based upon an evaluation, Illipsis Power is notifying the Commission of a 10 CFR Part 21 defect.
This report provides information as required by 10 CFR Part 21.21(6)(3).

Wescott, USNRC Region III, by telephone on December 7, 1981 of a potential 10 CFR Part 21 defect.

Gerstner, Executive Vice President of Illinois Power, by means of this report hereby informs the Commission ot a 10 CFR Part 21 defect.

General Electric termination modules (T-mods) with part number 137D7743G053.
(iii)
Supplied by General Electric Company, San Jose, CA.
Manufactured by ACL-Filco Corporation, Santa Ana, CA.
(iv)
T-mods are pre-manufactured subassemblies consisting of a terminal board and a connector receptacle mounted on a metal bracket.
The wiring from the terminal board to the connector is incorrectly wired on all T-mods with part number 137D7743G053.
There are 29 T-mods with this part number.
During normal testing in October, 1981, one T-mode was found incorrectly wired and was correct-ed.
Of the remaining 28 T-mods, 14 carry satety related circuits; therefore, the safety hazard created by this defect is that safety systems will not perform their safety functions.

Maintenance Work Request #A0434, completed 12/1/81, verified that the 28 remaining T-mods in question were incorrectly wired.

The defective T-mods are located on elevation 800 in Main Control Room as follows:

A Construction Work Request (5140) was issued on 12/10/81 to make the necessary jumpers to rewire the 28 T-mods.
This work was completed on 12/15/81.
Illinois Power Operations Maintenance Department has correctly rewired the T-mods in accordance with applicable GE wire lists utilizing Maintenance Work Request A-0434.
(viii)
The findings of the IP investigation into this matter have been discussed with General Electric.
GE has indicated that they will investigate this matter for potential applicability at other sites using similar equipment.
In Illinois Power's case, all circuits wired through these T-mods are tested in the field.
The circuits are checked for continuity from the PGCC device through the T-mod to the field device.
The circuits are also functionally checked prior to being released for op-erations, and therefore, just as the T-mod was dis-covered in October, 1981, to be incorrectly wired, so the remaining 28 would have been discovered during normal testing and checkout.
