Appendix A NOTICE OF VIOLATION The Detroit Edison Docket No. 50-341 Company Based on the inspection conducted on September 7-8, 1978, it appears that certain of your activities were in noncompliance with NRC require-ments, as noted below. This is an infraction.

10 CFR 50, Appendix B, Criterion XVI states, in part, that " Measures shall be established to assure that conditions adverse to quality, such as . . . deficiencies, . . . nonconformances are promptly identified and corrected."

The Enrico Fermi 2 FSAR, Section A17.1.16 states, in part, that " Edison

( has established and implemented written procedures to see that appropriate and prompt corrective action is taken when conditions adverse to quality are identified."

Contrary to the above, the deficiencies identified relative to the installation of safety related hangers had not been corrected from May to August, 1978. In addition, although there was a 100% rejection tate for all the hangers inspected, as of the date of this inspection, there were no plans for corrective action or other measures established to prevent recurrence. This condition continues to exist because procedural requirements specifying a time limitation, upon which corrective measures should be initiated, have not been established by the licensee contractors. Other circumstances permitting the subject condition to exist were determined to be: (1) the control of the QC hanger deficiency report had seemingly bypassed the licensee trend analysis system, and (2) the lack of licensee audits in the areas of hanger installations.
