ARIZONA PUBLIC SERVICE COMPANY (APS)

DESCRIPTION OF DEFICIENCY During a review of the safety-related instrument tubing clamps, it was found that locking devices were not installed on the threaded fasteners. The drawings contained in the Design Specification illustrated a typical fastener and specified that the nut shall comply with NF requirements. The individuals using the specification did not refer to the NF section of the ASME III Code; thereby,it was felt that the specification did not contain sufficient information for Construction to comply with ASME III NF4725 which requinms threaded fasteners to be provided with locking devices such as jam nuts, stop nuts, or drilled and wired nuts.

II.

ANALYSIS OF SAFETY IMPLICATIONS This condition is evaluated as reportable under the requirements of 10CFR50.55(e).

If the instrument tubing were to become disengaged from the seismic supports, damage to the tubing could render the safety-related instrumentation inoperative.

III. CORRECTIVE ACTION Design Change Packages ISJ-CH-089, 2CJ-CH-089, and 3CJ-CH-089 have been issued to have all the installed clamps corrected to comply with the Code. Specification 13-JM-715 has been clarified to provide definitive instructions concerning the threaded fasteners.

To preclude recurrence, the individuals responsible for these installations have received the revised specification and have l

reviewed the Code requirements in detail.
