Ayoendix A NOTICE OF VIOLATION Northern Indiana Public Service Docket No. 50-367 Company Based on the results of an NRC inspection conducted on August 18, 1978, it appears that certain of your activities were in noncompliance with NRC requirements as noted below. This item is an infraction.
10 CFR 50, Appendix B, Criterion V states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures or drawings..... ana shall be accc=plished in accordance with these instructions, procedures and drawings." Thatcher Engineering Corporation procedure 10-5 states, "All H-pile inspectors will be qualified as

Level I in accordance with the requirements of ANSI N45.2.6."
Contrary to the above, Thatcher inspection personnel had not received f
physical examinations for visual accuity nor had their employer certified their Level I inspector qualifications.
