Areas Inspected: Foundation piles installation, procedures and quality records; qualifications and training of pile driving inspectors. The inspection involved a total of 12 inspector-hours onsite by two NRC inspector Results: Three areas were inspected. One item of noncompliance was identified in one of the three areas inspecte (Infraction - failure to follow procedures relative to training and qualification of pile driving inspectors - Section II, paragraph 1.b).

Spessard, Chief Engineering Support Section 1 Review of Indicator Pile Program Records The purpose of the driving indicator pile program is to provide information to confirm the top of the bearing stratum into which production piling will be driven and for the purpose of confirming the estimated tip elevations for production piles under Category I main building complex. The inspector reviewed the pile driving records for the 49 piles driven between August 7, 1978 to August 17, 1978. The following table includes pertinent information such as, r pile identification, driving blow counts for the last 5 feet, 1 foot and 1 inch, estimated tip elevation from drawing Nos. S-48,

The ef fect of water jetting adjacent to these piles have effected the original estimated bearing stratum, The reason that piles SC47, 70 and 107 did not meet the acceptance driving criteria has not been determined at this tim No items of noncompliance were identified in the area inspecte . Observation of Pile Driving Work Activities l

l The inspectors observed the installation of one 14 HPil7 H-pile identified as AB-3 (auxiliary building) . The pile was driven using a Valcan 016 single action steam hammer with a rated energy of 48,750 ft-lb's using a 36 inch stroke.

l The pile was driven from elevation +8.0 with a final driving l

resistance of 523 blows per last 5 feet, 127 blows per last l foot and 11 blows per last 1 inch. This met the driving acceptance criteri ,

!* The pile cushion was measured after driving and found to be 4 7/8 inches. No acceptance criteria has been determined for the final cushion thickness.

Hayes, Chief Projects Section Review of Thatcher Engineer Corocration Pile Driving Procedures The inspector reviewed the following procedures relating to pile driving activities being performed by Thatcher Engineering Corporation, Procedure 7-1, Control of Stick and Wire Electrodes Procedure 10-2, Splice Welding Procedure 10-3, Pile Driving Procedure e Procedure 10-4, In-Lead Splice Inspection Procedure 10-5, H-Pile Inspection Personnel Qualification j and Training Procedure 10-6, Visual Weld Inspection Procedure 10-7, Cut-Off Inspection Procedure 11-2, Vertical Pile Load Test * Procedure 11-3, Lateral Pile Load Test Procedure 12-1, Instrument Calibration Procedure 13-1, Handling, Storage and Transporting Procedure 14-1, Inspection Plan Procedure 15-1, Nonconformance Reports Procedure 16-1, Corrective Action 4 The inspector reviewed training and qualification records of Thatcher Engineering Corporation inspection personnel. It was noted that four inspectors had not been certified to ANSI N45.2.6 Level I qualificatio Training and qualification records did not contain evidence that inspectors had been tested for visual accuity as regtf. red by Pro-cedure 10-5. This is an infractio (367/78-05-01)

Exit Interview The inspectors met with staff representatives (denoted under Persons Contacted) at the conclusion of the inspection on August 13, 1978.
