Revision 5' incorporates changes as transmitted in Reference 3 and 'ses guidance provided in Reference 4.

the large scale changes made, revision bars are not included in the right hand margins.
In a telephone conversation with' Mr. A. Bournia of the.NRC.on September 28, 1990, a one week extension was granted for this submittal.
-j Please note that several changes made in this program plan deviate from methodologies previously stated -in References'3, 5 and 7. 'These deviations are due to the results of further evaluations of specific' testing methodologies.
The changes submitted in this program plan either, meet the requirements of the American Society of Mechanical Engineers '(ASME)' Boiler and Pressure Vessel. Code Section XI and follow the-guldelines established in Reference 4; or, the appropriate Relief Requests have/are being submitted.
