The shift supervisor-and the controlt-room Senior Reactor 3
Operator (SRO) referred to McGuire Nuclear Station Technical
- )
. Specifications (TS) to evaluate the event.
They did not consult the Special orders document or the Justification.for-continued operation (JCO) document.- They concluded that~
l since the event only affected the B train VC inlet pathways, A train VC/YC remained operable, therefore no. notification was required.
Operations management was appropriately consulted and concurred with this decision.
3.
Corrective-steps which have been-taken and the results achieved..
(A).During the next several hours, operations management-and' Compliance management further examined the-event and concluded.that both trains of VC/YC wereEinoperable-1 (per theispplicable Special Order'and JCO.
The result of this was that TS 3.0.3 was in effect.for--
approximately 16 minutes'and-this was duly logged.
However, no NRC report was issued since power reduction was not started.. These actions were performed by a different shift' crew.
(B) LAfter several more hours and detailed examination of
-the event by. Operations and~ Compliance management,.it
-was determined that-this " event or condition alone could have-prevented the fulfillment of the-safety.

consequences of an accident".
The event was-immediately reported per 10CFR 50.72(b)(2)(iii)(d)-
which was more that six hours late of the required-four hour allowance.

This additional event was discussed with an SRO-representative from each operations shift crew on 9/14/90 at the monthly shift supervisor meeting

This'is scheduled for 9/28/90.

Operations. group managers will evaluate and implement enhancements to the JCO and Special Orders programs.
