140TICE OF VIOLATION Iowa Electric Light' and Power Company Docket tio. 50-331 Duane Arnold Energy Center License fio. DPR-49 During f4RC inspections conducted on August 24 through October 14, 1993, violations of fiRC requirements were identified. In accordance with the

" General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C, the violations are listed below:  !

1. Appendix B of 10 CFR Part 50, Criteria II, " Quality Assurance Program," +

requires, in part, that activities affecting quality shall be ..

accomplished under suitably controlled conditions. Controlled ,

conditions include the use of appropriate equipment, suitable .'

environmental conditions for accomplishing the activity, such as adequate cleanliness, and assurance that all prerequisites for the given i activity have been satisfied. -

Contrary to the above, the licensee failed to maintain adequate  !

cleanliness in the vicinity of the reactor vessel, as evidenced by tie '

wraps and wood found in the reactor cavity, during the period from -

August 3 to September 15, 1993, when the reactor head was removed from '

the reactor vessel.

This is a Severity Level IV violation (Supplement 1).

2. Technical specification 6.8.1 specified that written. procedures covering areas of normal startup, operation, and shutdown of systems and procedures be implemented,
a. Maintenance department procedure, Motor-G080-002, step 5.3.4(2), " General Electric High Thrust Vertical Induction Motors," required that the upper motor '

bearing be installed in the correct orientation.

Contrary to the above, on August 10, 1993, the "A" emergency service water pump motor upper bearing was not installed in the correct orientation. (50-331/93015-02a(DRP))

b. Maintenance department procedure, VALV0P-L200-007, "Limitorque Valve Operator Type SMB-0, SMB-1, SMB-2, SMB-3, SMB-4, and SMB-4T," step 5.3(14), required that.the clutch tripper fingers be -

installed in the correct orientation.

Contrary to the above, between August 14 and 17, 1993, the clutch -

tripper fingers for valve M0-2740 were not installed in the correct orientation. (50-331/93015-02b(DRP))

c. Design change package (DCP-1537) for the low pressure coolant injection (LPCI) "B" loop select logic, required that wire "BE-1" be connected to termination point "BE-13."

Contrary to the above, on August 14, 1993, wire "BE-1" was not '

connected to termination point "BE-13. (50-331/93015-02c(DRP))

d. Surveillance test procedure (STP) 41A006.2, "(Scram)

Discharge Volume High Water Level Instrument functional ,

Test / Calibration," step 4.1, required that only one channel be tested at a time.

Contrary to the above, on August 22, 1993, more than one channel  ;

was tested at a time when testing on half-scram channel "A-2" was not completed prior to beginning testing on half-scram channel "B-1". (50-331/93015-02d(DRP))

e. Fuel and Reactor Component Handling Procedure (FRCHP) Number 5,  !

" Procedure for Moving Core Components Between Reactor Core and Spent fuel Pool, within the Reactor Core, or within the Spent Fuel .

Pool," Attachment 1, specified the control rod blade guide at core location 10-27 be installed in a southeast orientation.-  ;

Contrary to the above, on September 5,1993, the control rod blade j guide at core location 10-27 was not installed in a southeast orientation. (50-331/93015-02e(DRP))

This is a Severity Level IV violation (Supplement 1).

hereby required to submit a written statement or explanation to the U.S. .

Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, f D.C. 20555 with a copy to the Regional Administrator, Region III, 799 [

Roosevelt Road, Glen Ellyn, Illinois 60137, and a copy to the NRC Resident Inspector at the Duane Arnold Energy Center, within 30 days of the date of the '

letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for F each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will-be taken to avoid further violations, and (4) the date when full compliance will be achieved. l If an adequate reply is not received within the time specified in this Notice, .

an order or a Demand for Information may be issued to show cause why the license should not be modified, suspended, or revoked, or why such other "

action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
