o WEEK ENDING SEPTEMBER 14, 1990 NRC Dbservation of INP0 Evaluation Team A member of the Performance and Quality Evaluation Branch (LPEB) is accompanying-

.an INP0 evaluation team at Point Beach.

Stewart D. Ebneter, Regional Adminis-trator, Region II, will observe the last.few days of the evaluation. NRC staff are observing the INP0 evaluation process in order to determine the emphasis

'l and quality of maintenance area evaluations. These observations are part of a broad staff assessment of the status of maintenance in the industry and the industry commitment to improving maintenance.

NCI Task Group Members of the Division of Radiation Protection and Emergency Preparedness attended a meeting of the Task Group on September 12, 1990 for the purpose of preparing a management briefing package regarding the National Cancer Institutes report, " Cancer in Po>ulations Living Near Nuclear Facilities."

International Visit on High-Level Waste Staff from the Division of High-level Waste Management and the Office of l '

I Governmental and Public Affairs met wich representatives from the French Nuclear Protection and Safety Institute, and the Central Service for Nuclear i

Installation Safety on September 11, 1990. Topics discussed included problems being encountered in the United States with the U.S. Environmental Protection Agency high-level waste standard and similar issues in France.

In addition, the French gave a presentation on the ongoing waste site inves-tigation activities. :This included a summary of the activities in the four different media being explored, clay, granite, salt, and schist. During the

' discussion, the French noted that the Prime Minister had placed a one year moratorium on any work on the high-level waste program. The Prime Minister's action was a result of local opposition c.t the four sites. Also discussed was the waste vitrification process in France, and the similarities and differences in the quality control techniques being employed by the French and United States glass producers. Finally, a discussion on quality assurance

-issues in the high-level waste area was held.

1989 Accident Sequence Frecursor Report The 1989 Accident Sequence Precursor (ASP) report, NUREG/CR-4674 Volumes 11 and 12, has been published and distributed. This report provides a quantitative evaluation of significant operating events described in licensee event reports during 1989. It provides a safety significance perspective of 1989 operating experience. There are no precursors in 1989 with conditional core damage probabilities higher than IE-3. There are seven precursors in 1989 with conditional core damage probabilities in the 1E-4 to /E-4 range, with the steam generator tube rupture at McGuire-1 (LER 89-004 having the highest conditional core damage probability (7E-4).

'I Nine Mile Point Units 1 and 2 Unit 1: Power Ascension Update The licensee-achieved criticality on 7/29/90. One forced-shutdown and two manual scrams occurred during the period. The forced shutdown on 7/30/90 was initiated to correct high-drywell leakage caused by a damaged electromatic l

relief' valve (ERV) downcomer vacuum breaker and two leaking ERVs.- The manual 1

scrams on 8/6/90 and 8/19/90 were initiated as a result of excessive vibration

!on the main turbine.

The unit is currently operating.just below 25% which is the upper limit of the first plateau in the-Power Ascension Program.- The licensee has completed all testing and other activities scheduled for Phase I of the Power Ascension i

Program.;. As required per Supplement I to Confirmatory Action Letter 88-17, e self-assessment of operation was presented to the NRC Restart Panel on 9/10/90 prior to increasing power to the next plateau (25% to 75%). After completion of' actions initiated by the. licensee, as a result of lessons

, learned from their self-assessment, the Unit will increase power beyond the y

The-NRO taff briefed the ACRS-on 9/7/90. The ACRS focused principally on

gaining an understanding of process used and the basis for the decision authorizing plant restart. Although no licensee presentation was made,

' licensee representatives at the meeting included Lawrence Burkhardt III,

. Executive Vice-President and his successor, Ralph Silvia.

-Unit 2:

The first refueling outage was entered three. days early on 9/4/90 as-e result

.of/a reactor trip.

Besides-routine refueling activities which include replacing 9 of 18 SRVs and replacing liners on the condensate demiM;alizer, 1the licensee is planning major modifications to both recirculation pumps.

Palisades' Plant

Palisadesiscoastingdown'(RCSboronconcentration21 ppm)inpreparation for'itstfive month Steam Generator Replacement and refueling outage. The

-outage commences on September 15.

There are approximately 500 additional personnel onsite in support of the outage. These contract personnel (a majority from Bechtel) will peak near 4

600 during the months of October and November. A modification to the protected 1 area security building has been completed and essentially doubles the processing capability. Construction of a temporary containment access facility.(except for the doorway opening to the fuel building) is also complete. Use of this, temporary facility will minimize contract personnel

" traffic" in the auxiliary building.

NRC-Observation of INPO Evaluation Team i

Meeting with BWR Owner's Group Emergency Procedures Committee On September 5,1990, the staff met with the Emergency Procedures Comittee i

(EPC) of the BWR Owner's Group to discuss the operation of nitrogen containment

~atmosphericdilution(NCAD) system. The staff expressed concerns that the NCAD system. which is a safety system and credited to meet 10 CFR 50.44 are not being)used for combustible gas control in the Emergency Operating Procedures (EOPs'.

lines (EPGs) after considering the pros and cons of the use of the system during an= accident. The staff indicated that additional technical efforts were needed

.to' quantify the-benefits as well as negative consideration of including the system 'in the EPGs.

Team Leader Counterpart Meeting

- Approximately 60 Regional and Headquarters inspectors and managers participated

in a team leader counterpart meeting September 4-6, 1990. The objectives of the meeting were to improve team leader skills, promote group identity, and i

identify team leader qualification and training requirements. Messrs. J. Sniezek,

-T.:Murley, and B. Davis gave senior NRC perspectives on the inspection process, team leaders, and the Regulatory Impact Survey. A number of important issues Lwere discussed during the meeting, including whether written inspector questions were acceptable, quality of.NRR team members, announcement of team inspections and multiple discipline teams to perform core inspection modules.

Public Meeting On September'6, 1990, the Director, DREP, participated with Regional Administrator, Region I, in a public meeting in Plymouth, Massachusetts.

The purpose of the meeting was to allow the NRC staff to hear directly from local officials who are involved in emergency preparedness planning around the Pilgrim Nuclear Power Plant. The information received will assist the staff l

in evaluating the continued progress in offsite emergency planning and prepared-

-i ness for Pilgrim.

In addition, DREP staff members EPB were viriting emergency response organizations in the vicinity of the Pilgrim site to collect current data for a report to the Comission on the status of offsite emergency preparedness around Pilgrim.

I Tactical Exercise at Nuclear Fuel Services (NFS) She On September 12, 1990, the licensee's Tactical Response Team conducted a full-scale force-on-force exercise at the NFS Erwin site. The exercise was observed by Headquarters and Region 11 personnel. The physical security upgrade rule for Category I fuel facility licensees requires the licensees to conduct quarterly

tactical exercises to assess the overall performance of their on-site security system. To date, five full-scale exercises have been conn eted at the Category I facilitits.

New Fuel Shipping Package l

No. UNC-2500 packaging. The packaging is to be used in the phase-out of UNC activities. An application is expected to be submitted to the NRC in September.

Northern States Power Company Prairie Island Independent Spent Fuel Storage Installation (15FSI)

The NRC has received an application from Northern States Power Company for a license to construct and operate a dry cask ISFSI at its Prairie Island Nuclear Generating Plant located in Red Wing, Minnesota. The proposed ISFSI l

- will accommodate a total of 48 casks and will use the TN-40 Dry Cask Storaae System designed by Transnuclear, Inc. The TN-40 cask will hold 40 Prairii Island fuel assemblies which have been decayed at least 10 years. The proposed ISFSI will provide adequate onsite spent fuel storage capacity to enable Prairie Island Units 1 and 2 to operate until the expiration cf their

respective operating--licenses in 2013 and 2014.-

This was )ursuant to the Agreement on Thermal-Hydraulic Research between the USNRC and tie U.S.S.R. Ministry of Nuclear Industry and Power and the Kurchatov Institute for Atomic Energy. This 3 1/2-year cooperative Agreement was signed on June 29, 1990. Per thic Agreement, the U.S.S.R. will provide code assessment results and thermai-t draulic test data. Through this Agree -

ment, the U.S.S.R. has become the 16th member of the USNRC International Code Assessment and Application Pregram (ICAP) which was established about five years i.go to obtain code assessment results from foreign participants.

An international conference to discuss probabilistic risk analyses and its limitations was held in Santa Fe, New Mexico, September 3-6, 1990.

to papers being presented by representatives from Italy, Germany, Sweden, ), E.

France and Japan, several NRC papers were also presented by T. Murley (NRR

.Beckjord (RES), and J. Merphy (RES). The meeting, which was organized and chaired by D. Ross-(AE00) with RES support, clearly demonst ated the broad acceptance of PRA methods and agreement that future work should focus on'such I

areas as. human reliability and common cause-failure. A summary report of insights from the papers and panel discussions will be published by CSNI in the near future.

' Oyster Creek Diagnostic Evaluation Planning and preparation for the Oyster Creek DET continued during the period September 10-14. On September 12, the designated Oyster Creek DET manager b_riefed the EDO and DEDR on the plans for the Oyster Creek DET including the areas of emphasis, schedule of major milestones, and anticipated team compc ition.

Incident Response Activities it. Support of State Programs On September 12, 1990, the Deputy Directer, AE00, participated in a panel

'discussionattheStateLiaisonOfficers'(SLO) meeting. Topics addressed were.

the role of the NRC during a response to an event, the upcoming Dose Assessment in Baltimore, Maryland, and the status of the Emergency Response Data

Following the SLO meeting, the Chief, IRB and staff members gave System a tour.and briefing on the NRC Operations Center including a demonstration of ERDS to the attendees.

Technical Training in Support of State Programs The TTC's Senior Health Physicist (Materials) participated in a GPA/SP sponsored Inspection Procedures Course in San Antonio, Texas, September 10-14,-1990.

Low-Temperature Overpressure Protection: Testing PORVs with the Alternate

The issue of inadequate nitrogen supply was first highlighted in Information Notice 82-45. Generally, the problem is caused by excessive pressure drop or 1eakage in the backup gas supply lines and no surveillance on stroke time with the backup gas supply.

Information Notice 89-32 addressed surveillance requirements for LTOP systems, but did not specifically include testing the l

backup pneumatic supply-system.

Suggestion was made to issue a supplement to the Information Notice 89-32.

Los's of Off-Site Power to Comply with NRC Regulations. AEOD/T90-12 During trouble shooting of the station auxiliary transformer, the transformer faulted and caused both reactor recirculation pumps to trip at Brunswick Unit 2, a boiling water reactor (BWR).

In accordance with NRC Bulletin No. 88-07,.

Supplement 1, " Power Oscillations in Boiling Water Reactors," Unit 2 was manually scramed although the licensee-knew the scram would also result in a loss of off-site power (LOOP) to Unit 2.

Since a loop is not a desirable transient, the circumstances of the event were investigated to determine if the regulatory bulletin may be responsibe for other undesirable transients at

.the BWRs. We found that no additional scrams had occurred due to licensee compliance with NRC Bulletin No. 88-07 or Supplement 1.

AE0D also_ concluded that the LOOP at Brunswick Unit 2 was not caused by lack of adequacy with the Bulletin, but by the licensee's failure to take all necessary actions'in response to the Bulletins.

' The class consisted of twenty-seven students - 15 from States (12-Agreement State; 3-non-Agreement State), 6 from NRC and 6 military.

- CRCPD Executive Board Meeting The Conference of Radiation Control Program Directors, Inc. (CRCPD) Executive

. Board met at NRC Headquarters on September 10-13, 1990 in Rockville, Maryland.

Chairman Carr met with the Board members to discuss issues of interest to the

National State Liaison Officers' Meeting o

- State Programs conducted a National State Liaison Officers' Meeting on September 11-12, 1990 in Rockville, Maryland. StateLiaisonOfficers,(SLO)are

. Radiography and Scrap Metal Seminar The. Institute of Scrap Recycling Industries, Inc. (ISRI) sponsored a seminar on L

- radioactivity in scrap metal on September 12, 1990 in Washington, DC. The purpose of the seminar was to provide a setting for the NRC, ISRI, and the

trade associations representing the major consumers of scrap metals to meet and

- consider the problem of radioactivity related to both ferrous and non-ferrous scrap. : Representatives from the Environmental Protection Agency and the Department of Transportation also participated in the seminar. Joel Lubenau, L

Concord, New Hampshire. The Committee members questioned NRC on control of multiple practices,_ dose models and proposed legislation being considered in New Hampshire to restrict BRC. The NRC witnesses indicated that if the legislation were enacted as written, it appears that all radioactive material (including already exempted items) would be required to be disposed in a licensed LLW disposal facility.

-volume and detail of data to have value for use independently of the-ourposes they-serve in the agencies. The study focuses on the availa-bility, accessibility and retention of such databases to support future research.

In conjunction with the NAPA study,;which is being done forLthe National Archives and Records Administration, Mr. Seymour-Greenstone visited the Nuclear Regulatory Commission on September 14,J1990, to begin efforts to identify ~ and build an inventory' of automated databases, and

define a set of criteria for appraising electronic databases. Mr.

S_ignificant Enforcement Actions 1.

A Order Imposing a Civil Penalty in the amount of $6,250 was issued on I

' September 13, 1990 to MQS Inspection, Inc., Elk Grove Village, Illinois.

The action was based on an overexposure event which resulted in four violations of NRC requirements. There violations were:

(1) exceeding the whole body dose limit during a calendar quarter,_(2) failure to i

survey the circumstances of the radiographic exposure device following an exposure,-(3)failuretoreporttotheNRCwithin24hoursaneventwhich threatens to cause exposure to the whole body of 5 rems or more of radiation,'and (4) failure to survey and lock the exposure device after each et,osure. The: licensee responded to the Notice in a letter dated May 9, 1990. The licensee denied Violation C'(reporting requirement),.

protested the imposition of the civil penalty and requested 100% mitigation.

After. careful consideration of the licensee's response, the staff has-concluded that the violation did occur-as set forth in the Notice of L

amount of $1,875 was issued September 14, 1990 to St. Luke's Hospital, L

Cleveland, Ohio. This action was based on a therapeutic misadministration that resulted from. failure to check a patient's planning chart prior to o

. administration. Additional violations of reporting requirements as to

. therapeutic misadministrations are cited without civil penalty. The base.

civil penalty was mitigated for identification and reporting and corrective action, but was escalated for past performance, with a net mitigation.of 25%.

Civil Penalties Paid 1

a 1.- ' Somat Engineering, Inc., Taylor, Michigan paid the civil penalty in the.

amount of.$125. The action was based on failure to maintain constant-contro's over a moisture density gauge.

In this case, an. employee made a conscious decision to' leave the gauge unattended in an unrestricted

area for-a'short time while he drove to a telephone to make a call.

U.S. Department of Agriculture, Washington, D.C. paid the civil penalty in the amount of $5,000. The action was based on fifteen violations that, L

L when viewed collectively, demonstrate a lack of management oversight.

Significant among these violations, was a failure to ensure that facilities 1

under this broad license were inspected internally by the radiation safety' staff at required frequencies. The violation was identified during NRC L

inspections in 1988 and 1989.

Completion of the systems furniture standards review by Dewberry & Davis' interior design' group has been delayed about two weeke. The delay, which is not critical to the overall project schedule, resulted from a decision by the developer's managing architect-for the TWFN. project to require a redesign of the proposed.proto-typical floor layout submitted by Dewberry & Davis. The Office of Consolidation needs the proto-typical floor layout to determine the space utilizatinn efficiency factor which,'in turn, will determineithe overall space. requirement in the building, and how the various organizational units and activities wit 1' fit onto the floors.

New Reg Cafe During the week, the contractor for the outdoor furniture, Architectural Precast,. Inc., told the Director, CONS, that the company had decided to completely; replace the-existing furniture with new, redesigned-equipment.

Company Vice1 President, David'Foresta,-estimates that the nine-tables with'

Response to New Hampshire State Legislature by NRC On September 13, 1990, in response to a request from the New Hampshire State Legislature, Fred Combs, Assistant Director for State, Local-&

Indian Relations, and Malcolm Knapp and John McGrath, of the NRC's Region 1 Office, addressed the New Hampshire House-Science, Technology and Ener0y committee-on the NRC's Below Regulatory Concern (BRC) policy.- The New

-Hampshire legislature is considering a bill that would prohibit disposal e

of certain BRC waste in that State.

Nine Mile Point Unit 1

)

On September 11, 1990, the licensee presented the results of their self-l assessment at the 25% plateau to the NRC staff Restart Assessment Panel.

The Niagara Mohawk assessment was generally consistent with that of the-Panel. The licensee plans to escalate power to the 75% plateau and con-tinue the power ascension test program on September 12,.1990. The restart inspection team plans to provide around-the-clock coverage l.

during the transition and significant testing activities.

Vermont Yankee Fuel Assemble' Failure 1

During the past operating cycle, Vermont Yankee ex)erienced fuel failures.

Using data collected during the operating cycle, tie licensee was able to

~

predict.the approximate number and locations of the failed fuel assemblies in the reactor. As part of a sipping, plan, all bundles % these suspect 1

areas have been tested by the licensee and 4 leaking fuel assemblies have been identified. Three of the four assemblies-have been in the reactor core for three operating cycles and are scheduled to be discharged to the i

spent fuel pool. The fourth assembly was new when installed during the February 1989 refueling. As part of the normal outage activities, General Electric was contracted to come in and, using an underwater periscope, perform visual ins)ections of some of the fuel.

In particular, GE was asked to look at tiose fuel assemblies which were determined to have failed i

and to determine the mechanism of failure. The inspection of the fourth assembly identified one of its 60 half inch diameter fuel rods as being s

failed. The. failure appears to'have occurred as a result of a manufacturing defect; however, further analysis by the fuel vendor is required to determine-the actual cause. The failed rod has a crack which has allowed some of the uranium pellets normally contained in the rod to erode.

On September 11, 1990, at 1:33 p.m., Sequoyah lost shutdown cooling for a

five winutes due to modification work (Eagle 21) in the Auxiliary Instru-j ment Room. Operators switched to a different pressure channel and restored a

shutdown cooling.

General Electric Company On September 13 the General Electric. fuel facility '.n Wilmington, North C3rolina conducted its, annual graded emergency preparedness exercise. The exercise was fully successful with two minor findings identified.

Carolina Power and Light - H. B. Robinson On-September 11, 1990, an Alert was declared.when workers cut through a' freon line in the Air Handling Room (beneath the control room) and had to 1

evacuate the area. Other areas of the plant were not affected. The toxic gas was disbursed and the Alert was terminated in one and a half hours.

The licensee made proper notifications to State and local officials. -The j

reactor was a shut down for refueling during the event.

Illinois Department of Nuclear, Safety On September 7,1990. Nr. H. 4. Miller, Director, Di',ision of Reactor Safety, and members of his staff met with the Illinois Department of

~L Nuclear Safety in Springfield, Illinois.

The purpose of this meeting was to discuss the scheduling of upcoming joint inspections under the Mec randum of Understanding (MOV) and the State draft regulations.

. Point Beach Nuclear Power Plant Approximately 150 dout,le crested corriorants have been found dead in the t

[

water intake forebay at the plant. These birds, which are a large migratory waterfowl, are a protected species.

Point Beach has a large doughnut shaped intake structure about 300 yards offshore that resembles a rocky island, which is a favorite roosting place for the birds.

In the process of diving for fish, they are possibly being drawn into the intake piping where they drown. This is the first such occurrence of these birds roasting at the plant. The licensee is taking action to remedy the 11tuatien. An air horn was installed at the intake structure to frighten off the birds, with limited success. A chain link fence is being considered to cover the intake structure.

Representatives from the State r

of Wisconsin Department of Natural Resources and the U.S. Fish and Wildlife Service are investigating this issue.

Perry Nuclear Plant On September 7, 1990, the plant's main turbine was removed from the grid connencing the licensee's second refueling outage.

Following main turbine overspeed testing, a reactor shutdown by control rod insertion was commennd. riowever, at about 10 percent reactor power, the licensee einted to complete the reactor shutdown by performing a manual scram du6 to a failure in the motor driven feed pump and increased turbine driven feed pump vibration at the low feed flow conditions. The plot responded as expected to the manual scram with reactor water level control maintained with the reactor cbre isolation cooling system. One anomaly N eurred while securing the turbine driven feed pumps when the "B" pump could not be tripped from the control room, in addition, when the eight main steam isolatiot valves (MSIVs) were closed by a " slow close" method, 5

MSlYs 22C & 28B reopened. Also, a reactor water cleanup system isolation L

occurred cu high delta flew. The isolation signai was reset and the cause is being investigated. Major work planned for thir outage includes refueling, replacement of all 19 safety relief valves replacement of reactor recirc pump shafts, and overhaul of an emergency diesel generator.

The outage is currently planned to last about 80 days.

Proposed Rule to be Signed by EDO On September 14, 1990, the Executive Director for Operations approved a prooosed rule 10 CFR 16 entitled " Salary Offset procedures for Collecting Debts Owed by Federal Employees to the Federal Government." The rule is necessary to conform NRC regulations to the Debt Collection Act'of 1982 which requires each agency to establish a salary offset program for the collection of these debts. The action is intended to allow the NRC to further impr6ve the collection of debts which are owed the Federal Government. Inis rule applies to all Federal employees who owe debts to the NRC and to current empl?yees of the NRC who owe debts to other Federal agencies.
