This letter refers to an announced NRC safety inspection conducted by Mr. T. Kenny on
September 17-21,1993, at Maine Yankee Atomic Power Station, to address clarification or
improvement items discussed in Table 1 of NRC Inspection Report No. 50-309/92-81.

The inspector reviewed procedures, representative records, interviewed personnel, and
overall observed areas important to public health and safety. The inspection focused mainly
on the motor-operated valve (MOV) program and its implementation.

The inspector noted that the Generic Letter 89-10 MOV program is continuing to progress
toward completion by June 30,1994. The inspector also witnessed a good level of
management involvement in the implementation of the MOV program. However, based on

the results of this inspection, certain of your activities appeared to be in violation of NRC
requirements, as specified in the enclosed Notice of Violation (Notice). The violation is of
concern because, in this case, the failure to follow procedures resulted in damage to the

safety-related motor-operated valve HSI-M-42. You are required to respond to this letter and
should follow the instructions specified in the enclosed Notice when preparing your response.
In your response, you should document the specific actions taken and any additional actions
you plan to prevent recurrence. After reviewing your response to this Notice, including your.'

proposed corrective actions and the results of future inspections, the NRC;will determine -
whether further NRC enforcement action is necessary to ensure compliance with NRC
,
regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and

its enclosures will be placed in the NRC Public Document Room.
