Dear Mr. Liu:
This refers to the region based team inspection of engineering and technical
support led by Mr. Clark Vanderniet and others of this office conducted
September 7 through October 1, 1993. The inspection included a review of
authorized activities for your Duane Arnold Energy Center (DAEC). At the
conclusion of the inspection, the findings were discussed with those members
t
of your staff identified in the enclosed report.
The purpose of this inspection was to assess the quality and effectiveness of
engineering involvement in plant activities. The inspection consisted of a
review of modifications to the High Pressure Coolant Injection (HPCI) and
Stand-By Liquid Control (SBLC) systems during the recent refueling outage; a
review of active temporary alterations, deviation reports, engineering work
requests, engineering maintenance actions, and relevant procedures; and
interviews with plant staff.
Overall, the team concluded that the extent and quality of engineering and
'
technical support activities performed by the staff at the site was sound.
However, the team determined that many administrative controls, detailed in
the administrative procedures governing engineering and other activities, were
not being fully implemented. Most notably, the formal Procedure Change
Notices and Engineering Work Requests, programs and process's had almost
entirely been supplanted by informal practices . implemented by subordinate
.
personnel.
While no specific examples were identified where the informal
practices affected safe facility operation, we are concerned that senior site
,
management appeared not to be aware of the length and breadth- of the problem.
Senior site management needs to thoroughly review administrative controls and
determine which controls are ineffective and need to be changed and which are
applicable and need to be executed.

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The first violation concerns the failure to take timely-
corrective actions with two examples. The second violation concerns the
failure to follow approved plant procedures with multiple examples.

take to ensure the appropriate level of management involvement.

response, please address the specific actions which will be taken to correct

the violations cited in this report.
You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence. After reviewing your response to this
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790 of the Commission's regulations, .a copy of
this letter, its enclosures, and your response to this letter will be placed
in the NRC Public Document Room.

subject to the clearance procedures of the Office of Management and Budget as
