SYSTEMATIC ASSESSMENT OF LICENSEE PERFORMANCE (SALP)
(NRC INSPECTION REPORT N0. 50-321/93-23 AND 50-366/93-23)
The NRC Systematic Assessment of Licensee Performance (SALP) nas been completed for Hatch Nuclear Plant. The facility was evaluated for the period of May 31, 1992, through November 27, 1993. The results of the evaluation are documented in the enclosed SALP report. This report will be discussed with you at a public meeting to be held at the Hatch site on January 21, 1994, at 1:00 p.m.
This SALP was conducted under the revised SALP process implemented by the l
Nuclear Regulatory Comrc.ission on July 19, 1993. The revised process assesses i
four functional areas: Operations, Maintenance, Engineering and Plant Support. The Plant Support area includes: radiological controls, security, emergency preparedness, fire protection, chemistry, and housekeeping controls.
i Although the performance of Hatch was good in the Plant Operations and Maintenance areas, there was a decline in both of these functional areas. The performance in the Engineering functional area improved. The improving trend noted in the last two SALP periods for the former functional area of Raaiological Controls was sustained, and when combined with the continuing strong performance in emergency preparedness and security, resulted in a superior rating in the functional area of Plant Support.
Contributing to the decline in the Plant Operations functional area was inadequate operator performance and weaknesses in attention to detail by Operations personnel, resulting in unnecessary transients. Operator i
performance problems were noted early in the assessment period.
Although operator performance improved during the last few months of the period, this l
Improvement did not compensate sufficiently for the performance problems which occurred earlier.
In the Maintenance area, deficiencies in testing and poor work practices resulted in many challenges to plant reliability and availability, and several resulted in unnecessary scrams and transients.
Problems with surveillance, post-modification, and post-maintenance testing were significant to the assessment of declining performance.

equipment problems that caused operational problems.
In summary, these challenges indicated that maintenance was no longer functioning at a superior level.
The functional area of Engineering improved. Almost every aspect of engineering and technical support operated at a superior level. Onsite and
)
offsite engineering groups strongly supported plant operations.
Effective and professional communications between NRC and your staff were also noted.
In Plant Support, the positive effects of your ALARA program initiatives were key to the improved performance in radiological controls. When this was considered with the continuing superior performance in emergency preparedness and security, the new functional area of Plant Support was found to be excellent.
Your performance in self-assessment was mixed. Management involvement in the control room performance issue produced positive results, albeit after the problems had continued for some time. When emergent issues were raised to the level of the Event Review Team, there was usually an effective resolution realized. Also, training was noted as effective and superior in most areas.
Conversely, the failure to address issues effectively, such as the single failure vulnerability of the intake structure ventilation systems and the separation of the turbine building service water from other service water during accident conditions, did not indicate a superior level of commitment in the self-assessment area.
Your assessments failed to detect the declining j
performance in the Operations and Maintenance areas early in the assessment period.
In accordance with Section 2.790 of the NRC's " Rules of Practice," a copy of this letter and its enclosure will be placed in the NRC Public Document Room.
