Dear Mr. O'Connor:
This letter forwards the Diagnostic Evaluation Team Report for ' Zion Nuclear Station. The team assessed the effectiveness of licensed activities performed -
by Ceco in achieving safe operation of Zion and determined the causes of a,
performance deficiencies. The team of 15 evaluators, led by an NRC manager, was onsite at the Zion station during June 4 through-15 and June 25 through-
- 29. Team members also visited the corporate offices in Downers Grove, Illinois during these periods.
Findings were discussed with Mr.-Bide Thomas, a
President of CECO and Ceco staff at an exit meeting in Downers Grove on' August-14, 1990.
To achieve an independent perspective, the team was staffed with members who I
had no previous responsibility for the regulation of CECO.
Safety performance i
was evaluated in the areas of operations, maintenance and testing and the t
effects of management and engineering support on this performance were assessed.
l The team identified performance deficiencies in the areas of operations, j
maintenance and testing and found that weaknesses in management and j
engineering support had contributed to these deficiencies..Specifically, the i
team found that management control of operator activities and teamwork were weak; the operator training program was weak; safety related systems were poorly maintained; and, testing of safety related components had not been adequate.
Many of these deficiencies had existed for several years and most had been I
previously identified by your staff.
Corrective actions had been scheduled'in the Zion Performance Improvement P?an, but the amount of work needed for correction appeared to be more than could be fully accomplished in a timely.
manner by the station staff.
The resources needed had not been determined :
and corporate support had not been planned beyond that committed for normal plant operation and maintenance.
Although your staff had recently Mentified performance deficiencies similar r
to most of those found by the team, and had developed coarehensive corrective action plans, corrective actions had not fully addressed root causes-and progress tward resolution of deficiencies had been slow.

Overall, the fact that you had pr6viously identified.many of the deficiencies found by the team reflects aositively upon your self-assessment capability and; the corrective actions you lave initiated show an acknowledgement of the need for improvement. However, the slow rate of improvement indicates the need for increased management attention and support to address root causes and to-produce more-timely resolution of performance deficiencies.
It is important that you and Ceco management carefully review:the enclosed.
report, with special emphasis on the: areas requiring additional management-attention.
Following your review, I request that you determine the actions needed to assure timely resolutionLof performance deficiencies and to address root causes.
I also request that' you provide my office with.your-plans for addressing root causes, including your plans'for strengthening engineering, within 60 days-of the Cate of this letter.
In accordance with 10 CFP. 2.790(a), a Leopy of this letter and' the enclosure will be placed in the NRC Public Document Room.
