SUBJECT: N'RC Combined Inspection Report Nos. 50-352/93-23; 50-353/93-23
This letter transmits the findings of an announced inspection of the radiological controls program
conducted at the Limerick Nuclear Generating Station by Mr. R. L. Nimitz of this office on
September 7-10 and 14,1993. The findings of the inspection were discussed with Mr. R. Boyce
and other individuals on September 14, 1993.
This letter also transmits the summary of the Management Meeting held at the NRC Region I
Of6ce, King of Prussia, Pennsylvania, on September 8,1993. The meeting was held to discuss
your proposed receipt of fuel from the Shoreham Nuclear Generating Station. The meeting was
attended by Mr. D. Helwig and other representatives of Philadelphia Electric Company,
representatives of Long Island Power Authority, and members of NRC management and staff.
The meeting was open to the public and members of the public were in attendance. NRC
management stated that, based on the meeting and previous inspections, an appropriate level of
planning and preparation was provided to safely transfer the fuel from the Shoreham Nuclear
Generating Station to the Limerick Nuclear Generating Station.
This letter also acknowledges receipt and review of your July 30,1993, letter which transmitted
your evaluation of the January 27,1993, Unit 2 traversing incore probe (TIP) event.
Areas reviewed during the combined inspection were important to health and safety and included
action on previous findings; radiological controls program enhancements; quality assurance
oversight activities; radiological controls performance during the 1993 Unit 2 refueling outage;
the contamination control program including the circumstances surrounding the identification of
high levels of sodium-24 (Na-24) contamination identified in ' Unit 2 on August 20, 1993;
radiological controls during the unscheduled Unit 1 outage; and preparation and planning for the
proposed receipt of fuel from the Shoreham Nuclear Generating Station.

Overall, the results of the inspection indicated that an appropriate level of management attention
was directed towards understanding the root causes of the radiological controls events that
occurred in late 1992 and early 1993. The inspector identified that appropriate efforts were on-
going to enhance training and improve procedures in response to the events. In addition, the
inspector noted that actions were taken to improve the effectiveness of evaluations of self-
identified events in order to further enhance performance in the area of radiological controls.

Future inspections will evaluate the effectiveness of these performance enhancement initiatives.
Your efforts to conduct an independent evaluation of the January 27, 1993, TIP event were
considered a very good initiative. Your evaluation received an independent review by NRC
Headquarters. Contrary to the conclusion reached by your contractor, based on our review we
continue to believe that the violations transmitted with our April 9,1993, letter were
,
appropriate. We note that these violations were closed during this inspection.

Regarding specific findings identified during the inspection, our review indicated that very good
radiological controls were implemented for the unscheduled Unit 1 outage; QA oversight was
very good; no unplanned exposures or intakes occurred during the August 19,1993, Na-24
'
contamination event; and good planning and preparation was on-going for the receipt o" fuel
from Shoreham. One non-cited violation was identified which involved identification, by your
staff, of instances of contaminated material outside the radiological controlled area. The
contamination was low-level. Your staff took appropriate corrective actions for this matter. As
a result, and consistent with the guidance of the NRC's Enforcement Policy, this violation is
non-cited.
No response to this letter is required. Your cooperation with us in this matter is appreciated.
