SUBJECT: COMBINED INSPECTION REPORT NOS. 50-352/93-22 AND 50-353/93-22
This letter transmits the results of the resident safety inspection by Messrs. N. Perry, and
T. Easlick, from August 17, through September 25,1993, at the Limerick Generating
Station, Limerick, Pennsylvania. The inspection focused on issues important to ensuring the
public health and safety and consisted of observations of activities, reviews of documents and
interviews with plant personnel. The results are documented in the enclosed report and were
discussed with Mr. R. Boyce of your staff at the conclusion of the inspection.
Based on the results of this inspection, certain of your activities appeared to be in violation
of NRC requirements, as speciGed in the enclosed Notice of Violation (Notice). The
violation is of concern because, during the past year, other weaknesses have been identified
in the radiological controls area, by the NRC and your staff. Although we have observed
improvements and initiatives to corrective these weakness, we believe that additional
corrective actions may be necessary to ensure that weaknesses are identified and
appropriately addressed.
You are required to respond to this letter and should follow the instructions speciGed in the
enclosed Notice when preparing your response. In your response, you should document the
specific actions taken and any additional actions you plan to prevent recurrence.
Additionally, in two instances, we have exercised enforcement discretion because of your
identi6 cation of an event, your prompt and comprehensive corrective actions and you met all
the criteria of 10 CFR 2, Appendix C, Section VII B2. One instance involved the
inadvertent actuation of an underfrequency relay associated with the Unit I reactor protection
system power supply. The other instance involved the mispositioning of several emergency
service water valves.
