(NRC INSPECTION REPORT NOS. 50-327/93-37 AND 50-328/93-37)
This refers to the inspection conducted by D. Charles Payne of this office on
November 15-19, 1993. The inspection included a review of activities
authorized for your Sequoyah Nuclear Plant. At the conclusiori of the
inspection, the findings were discussed with those members of your staff
identified in the NRC Inspection Report, Enclosure 2.
Areas examined during the inspection are identified in the repor.. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress. A Simulator Facility Report is included in this
report as Enclosure 3, and a list of documents reviewed during the inspection
is included as Enclosure 4.
Based on the results of this inspection, certain of your activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). The violations are of concern because a properly
proceduralized licensed operator requalification program and proper
documentation of program activities are necessary to ensure program viability.
The ability of training department evaluators to detect weaknesses in operator
performance and properly analyze the weaknesses was noted as a program
strength. We are concerned, however, about the type and frequency of
weaknesses demonstrated by your operators during their simulator examinations.
Weaknesses were in communications, event diagnosis, understanding of control
board indications, and Emergency Operating Procedure usage.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to take to prevent recurrence. After reviewing your response
to this Notice, including your proposed corrective actions and the results of
future inspections, the NRC will determine whether further NRC enforcement
action is necessary to ensure compliance with NRC regulatory requirements.
