.s Gentlemen:

The following additional information is provided as requested by the referenced j

letter. As described below, the fuel movement in question was being governed-by-two station procedures.

E Nuclear Performance Procedure 10.25.1, Refueling - Core. Reload,'.provides-precautions, limitations, administrative limits, and instructions associated with the refueling evolution. One of the precautions.for'lcading fuel into;the. core j

is that-all. control rods be inserted. Also, fuel movement is' required'to,be j

controlled by the Special Nuclear Materials'_(SNM) transfer' form which'is completed in accordance with Procedure 10.21, Special Nuclear Materials Control and Accountability Instructions. The. initial SNM transfer form was completed in accordance with Procedure 10.21 and, recognizing the need to have all controls.

rods inserted, required that the withdrawn control rod be reinserted prior.to reloading the four removed fuel assemblies.

However, when it was recognized that two additional fuel assemblies had to be

'l removed in order to align the fuel support piece, an on-The-Spot Change-(OSC) was i

initiated in accordance with Procedure 10.21.

The OSC allowed the removal and replacement of the two additional assemblies without recognition of the Technical Specifications requirement as referenced in Procedure 10.25.1.

As described in our August 5, 1993, submittal, this procedural weakness will be corrected by.

l December 31, 1993.

.y Additionally, please be advised that in reviewing our response.to the Notice of

.1 Violation, we determined that the commitment regarding corrective action to be taken associated with licensed operator. training requires clarification.

In our.

I response, we stated that the circumstances associated with LER.93-023 had'been incorporated in a' Training Lesson Plan..In fact, the LER has been included in'a Training Lesson Plan as a reference, notfas a specific' event to be reviewed.
