NRC INSPECTION REPORT 50-298/93-26 (NOTICE OF VIOLATION)
This refers to the inspection conducted by Messrs. R. A. Kopriva and W. C.
'
Walker of this office on October 10 through November 20, 1993.
The inspection
,
included a review of activities authorized for your Cooper Nuclear Station
facility.
At the conclusion of the inspection, the findings were discussed
with you and those members of your staff identified in the enclosed report.
,
Areas examined during the inspection are identified in the report. Within
>
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The results of this inspection are documented in
pages 1 and 2, in the enclosed report.
,
Based on the results of this inspection, certain licensed activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). The violation is of concern because it identified a
situation where the shift supervisor authorized work to be performed on a fire
door and did not consider the t(fect of the work on the operability of the
control room envelope. We are also concerned that your maintenance planning
process did not identify the f act that the removal of Fire Door H-305 for
repair resulted in a breach of the control room isolation envelope on
November 18 and 19, 1993.
One unresolved item was identified in Section 2.1.
The item concerns the
implementation of the predictive maintenance program to promptly identify
degraded equipment.
The staff will review the adequacy and implementation of
this program during a subsequent inspection.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence. After reviewing your response to this
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will detarmine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.

In addition to the response directed by the Notice of Violation, you are
requested to address the weaknesses in your maintenance planning process which

resulted in the breach of the control room envelope on November 18 and 19.
Your response should address the reasons for the maintenance planning
j
weaknesses and the actions you have taken or plan to take to prevent
recurrence of this type of failure.
The responses directed by this letter and the enclosed Notice are not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of.
this letter and its enclosures will be placed in the NRC Public Document Room.
