This refers to the inspection conducted by E.-D. Testa of this office on-
November 29. -' December 3,1993, and a subsequent telephone discussion with a
,
member of your staff on December 14, 1993. The inspection included a review
.;
of activities authorized for your Crystal River facility. At the conclusion

of the inspection, the findings were discussed with those members ~of your
i
staff identified in' the enclosed report.

Areas-examined during the inspection are identified in the report. Within
'!
these areas, the inspection consisted of selective examinations of procedures
1
and representative records, interviews with personnel, and observation of
i
activities.in progress.

The enclosed Inspection Report identifies activitles that violated NRC
requirements that will not be subject to enforcement action because the'
licensee's efforts in identifying and/or correcting'the violations meet the_
.
criteria specified in Section VII.B. of the Enforcement Policy.

' We are concerned about the multiple examples of failure to follow procedures.

We consider these possible precursors to more safety .significant procedural

adherence problems in the: radiological controls area as well as potentially
other plant disciplines.
Also of concern were the' multiple missed '
opportunities to detect-and prevent slightly radioactive contaminated piping
[
from exiting the Radiation Controlled Area.
