From the summarized Notice of Violation, dated August 5, 1993, "a

licensee radiographer did not perform an adequate survey".

In

actuality it was an assistant radiographer that did not perform

the proper survey.

This does,not change the fact that the proper

survey was not completed,

and that the responsible licensed

radiographer at the site should have made sure the proper survey

was made.

In the above :ase the reason seems to be laziness on the part of

the radiographer.

He stated they didn't - know why the proper

survey was not performed, but said he did understand the entire

circumference was supposed to be surveyed after each radiographic

exposure.

The reason

for the violation was

the

assistant

'

radiographer and radiographer not following proper procedures.

During the 3rd quarter internal inspection I talked with the

radiographer and all other radiographers and assistants about the

violation,

cause,

and what needed to be done so no future

'

violations are made.

We went over the Operating & Emergency

Procedures,

physically demonstrated the survey of the entire

circumference

on

the

exposure

devise

and

cuestioned

the

radiographer on other areas of interest pertaining to surveys.

The importance of this survey (circumference around the exposure

devise and source guide tube), all other surveys and personnel

following O & E procedures were stressed during the inspection.

Future quarterly internal inspections will place more emphasis on

surveying the entire circumference of the radiographic exposure

devise and the source guide tube, with actual demonstrations.

Importance of all other surveys along with other inspection

requirements.

All new assistant radiographers and radiographers

will be required to demonstrate proper surveying techniques, if

the individual needs additional training, they will be required

to document the training prior to radiographic work.

comply

conditions

Arctic

Slope

Inspection's Operating and Emergency Procedures .for the radiation

safety program.
