ATTACHMENT 1
VIOLATION:
Title 10 CFR 50, Appendix B, Criterion XVI, " Corrective Action," requires
that measures shall be established to assure that conditions adverse to
-
quality are promptly identified and corrected.
Contrary to the above, on July 9, 1993, the NRC identified that a condition
adverse to quality had existed since July 9, 1992, which had not been
corrected. The condition concerned inadequate controls for monitoring vital
switchgear room temperatures to ensure that the design basis maximum
,
temperature is not exceeded.
Thi: is a severity level IV violation-(Supplement I).

RESPONSE:
GPUN concurs with the violation as stated.
The violation occurred when guidance issued via memo to mitigate temperature
concerns was not implemented. Consequently, a heat wave which occurred in
early July,1993, caused temperatures in both switchgear rooms to reach 100*F.
Immediate actions were taken to reduce and monitor switchgear room
temperatures to prevent exceeding the 104*F design basis value for electrical
components.
A contributing cause to this violation was the fact that engineering / technical
guidance was issued via memo rather than using a controlled work
request / tasking / tracking system such as the Technical functions Action Item
system.

Corrective Actions and Results Achieved:

The high temperature setpoint of the temperature alarm for the 480V Switchgear
Room has been changed from 105*F to 104*F, Additionally, the alarm response
procedure for 480V switchgear room high temperature has been reviewed to_

incorporate manual corrective actions outlined in engineering memo #5310-93-
045. Thermometers have been installed in the 480V Switchgear Room and the
Reactor Building Summer Tour Sheet has been revised to include monitoring room

temperature twice per shift.

In the 4160V Switchgear Room, thermometers have been installed and the
Turbine Building Summer Tour Sheet has been revised to include monitoring room
temperature twice per shift. During the July heat wave, the roll-up fire
i
barrier doors between the A&B and C&D 4160V busses were raised in accordance
r
with engineering memo #5310-93-165 to reduce the high temperature condition in
the C&D bus fire barrier enclosure. As a result, room temperatures did not
exceed 100*F.
!
To address the concern of engineering / technical guidance being issued via
memo,
a directive has been issued by the Site Director advising personnel to
use the Technical Functions Action Item Tracking System, in lieu of a memo, in
instances where action is requested on the part of the Receiving Department.

These actions are complete.

Corrective Actions to be Taken:

Procedure revisions will be made to include guidance to reduce Switchgear Room
temperatures such as opening fire barrier doors, using portable fans and
opening security doors. Additionally, the use of portable fans with flexible
ducting will be investigated as a viable solution to high temperatures in the
switchgear rooms.
Date When Full Compliance will be Achieved:
Full compliance was achieved on 7/9/93 when the above corrective actions were
implemented to monitor and alleviate switchgear room temperatures. The
additional corrective actions specified above shall be completed by 4/1/94.
