SUBJECT:

NRC INSPECTION REPORT 50-313/93-09; 50-368/93-09 (NOTICE OF

VIOLATION)

This refers to the inspection conducted by L. Smith, and other inspectors of

-

this office, on October 3 through November 13, 1993. The inspection included

a review of activities authorized for your Arkansas Nuclear One, Units I and

2, facility. At the conclusion of the inspection, the findings were discussed

with you and those members of your staff identified in the enclosed report..

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress. The purpose of the inspection was to determine

whether activities authorized by the license were conducted safely and in

.;

accordance with NRC requirements.

Based on the results of this inspection, certain licensed activities appeared ~

to be in violation of NRC requirements, as specified in the. enclosed Notice of

Violation (Notice).

The first' violation is of concern because a speed switch

was not replaced on an emergency diesel generator at the end of its apparent

recommended life.

The second violation involved the failure to maintain

postaccident sampling system valves in the position required by the procedure.

This violation is of concern because it indicates a lack of control of plant

equipment status.

Two violations, discussed in Sections 2.1.2 and 3.6 of the enclosed report,

are not being cited because the criteria in Paragraph VII.B of Appendix C to

'

10 CFR Part 2 of the NRC's " Rules of Practice" were satisfied.

Your staff

took prompt and effective actions to correct these violations.

It is our

understanding that, during the exit on November 18, a member of your staff

committed to certain corrective actions. These actions are listed in

Sections 2.1.2 and 3.6 of the enclosed report.

commitments differs from ours, please contact us immediately.

One unresolved item was identified concerning the failure to implement a

vendor recommended modification to reactor coolant pump seal packages on

Unit 1.

Further inspection will be performed to-determine whether design

change controls were properly implemented. This matter is discussed in-

Section 3.5 of the enclosed inspection report.

You are required to respond to this letter and should follow the instructions

j

specified in the enclosed Notice when preparing your response.

In your.

response, you should document the specific actions taken and any. additional

actions you plan to prevent recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

!

inspections, the NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.

this letter and its enclosures will be placed in the NRC Public Document Room.

The responses directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget as required
