SUBJECT:

NRC INSPECTION REPORT 50-482/93-24 (NOTICE OF VIOLATION)

This refers to the inspection conducted by Mr. G. A. Pick and the inspectors

identified in the attached report for the period of August I through

September 11, 1993. The inspection included a review of activities authorized

for your Wolf Creek Generating Station facility.

At the conclusion of the

inspection, the findings were discussed with you and those members of your

staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress.

The purpose of the inspection was to determine

whether activities authorized by the license were conducted safely and in

accordance with NRC requirements.

Based on the results of this inspection, certain licensed activities were in

violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice).

The violation is of concern because the failure to

provide cooling water flow to a safety injection pump highlights continued

operator weaknesses.

Another indicator of operator performance weakness

occurred when operators failed to notice that the steam dumps had armed for a

,

period of 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> 25 minutes.

Also, operations personnel improperly

'

implemented a clearance order that resulted in collapsing a condensate ammonia

supply tank.

Based on previous discussions, you are planning to discuss the results of a

recent internal assessment of operations during the Performance Enhancement

Program status meeting scheduled for October 22, 1993.

During this meeting,

you should also plan to discuss the recent indications of performance

weaknesses in the operations area, including your corrective actions.

Although 10 CFR 2.201 provides for a written statement of explanation to this

,

office after within 20 days of your receipt of this Notice, we note that this

violation had been corrected and those actions, including your root cause

analysis, were reviewed during this inspection.

Therefore, no response with

respect to this matter is required.

During this period your personnel determined that they failed to post

information in the time frame required by 10 CFR 19.ll(e). This violation is

not being cited because the criteria in paragraph VII.B.2 of Appendix C to

10 CFR Part 2 of the NRC's " Rules of Practice," were satisfied.

This

violation was an isolated occurrence and your staff took prompt and extensive

actions to correct the problem.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

this letter and its enclosures will be placed in the NRC Public Document Room.

Should you have any questions concerning this inspection, we will be pleased

to discuss them with you.
