i This is to inform the Commission that a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $25,000
(
will be issued on or about December 7, 1993 to.Entergy Operations Inc.
This action is based on two violations associated with the l
inoperability of one of the two trains of containment spray due to a header isolation valve that was unable to open upon demand i
when high differential pressure existed across the valve.
The violations have been classified as a Severity Level III problem.
The. base civil penalty of $50,000 has been mitigated 50 percent l
l after partially offsetting mitigation for identification and l
corrective action with escalation for prior opportunities to j
identify the valve's condition.
l It should be noted that the licensee has not been specifically I
informed of the enforcement action.
The schedule of issuance and i
notification is:

The State of Louisiana will be notified.
The licensee has thirty days from the date of the Notice in which l
to respond.
Following NRC evaluation of the response, the civil l
penalty may be remitted, mitigated, or imposed by Order.
