Dear Mr. Humphreys:
This letter addresses the NRC inspection led by Mr. J. J. Petrosino of this office on March 12 through 15, 1990, of your San Diego, California, facility and the discussions of our findings with Mr. T. Erickson, Mr. T. Roth and other members of your staff at the conclusion of the inspection.
The inspection evaluated a low voltage shutdown circuit problem identified by the San Onofre nuclear generating station (SONGS) in 1986 and reviewed the corrective actions you have taken in response to our June 3,1988, letter.
The s)ecific areas examined during the inspection and our findings are discussed in-tie enclosed report. This inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspector.
The inspection concluded that the implementation of your quality assurance 4
program failed to meet certain NRC requirements. The most significant inspec--
tion finding concerned your failure to effectively carry out your corrective actions regarding previously identified violations of 10 CFR Part 21.
These actions involved performing evaluations of deviations or informing applicable customers of deviations pursuant to 10 CFR Part 21. The specific inspection findings and references.to the pertinent requirements are identified in the enclosures to this letter.
The enclosed Notice of Violation is sent to you pursuant to the provisions of Section 206 of the Energy Reorganization Act of 1974. You are required to submit to this office within 30 days from the date of this letter a written statement.containing:
(1)~a description of steps that have been or will be taken to correct these items; (2) a. description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions
'and preventive measures were or will be completed. We will consider extending the Lresponse time if you can show good cause for us to do so. You are also requested to submit a similar written statement for each item which appears in the enclosed Notice of Nonconformance.
The responses requested by this letter are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
