Uninte ional Overexposure of Intant's Thyroid (Update)
Region V staff, NMSS and an NRC Medical Consultant have completed the onsite inspection and are continuing to evaluate an incident at the Tripler Army Medical Center in Honolulu, Hawaii. A nursing mother was given a 4.89 millicurie dose of iodine 131 on June 19, 1990 and a substantial fraction of that dose was passed to the mother's nursing infant resulting in a thyroid gland ablation dose to the intant.
Extensive dose assessment by the Oak Ridge National Laboratory utilizing data from whole body and thyroid scans and analysis of two breast milk samples indicate a dose to the intant's whole body of 17 rem and 30,000 rem to the thyroid gland.

DUANE ARil0LD REQUAL PROGRAM UNSAT NRC has found the Duone Arnold Requalitication training program unsatisfactory as a result of requalification examination given during the week of June 26, 1990.
Sixteen operators (4 crews, 3 operating and 1 administrative) were examined by the NRC. Three crews (2 operating and 1 administrative) failed the dynamic examinations. The operating crew composition included 4 SR0s. There were 4 individual failures (3 dynamic simulator exam and 2 job Performance Measures).
One individual tailed both the JPM and the simulator section.

GRAND GULF NUCLEAR STATION Entergy Operations, Inc., the licensee for Grand Guit, advised that the Governor of Mississippi signed a bill on Wednesday, June 27, 1990, which defines how tax revenues from urand Gulf will be distributed to counties in Mississippi served oy Mississippi Power & Light Company. This law supersedes a previous law wnich Claiborne County contested in a lawsuit as not properly apportioning tax revenues to the County and the City of Port Gibson.
ihe present law was the result of a compromise between officials of the County and City with the Governor and Legis-lature of the State.
These local government bodies now agree there will be acequate funds for the performance of functions required by their emergency response plbnts in the event of a radiolog1 cal emergency at Grand Gulf. During the controversy, the licensee had arranged backup emergency plans with the State.

will cons 1 der both "ATWS" and "beyond ATWS" (e.g., no boron injection activation) and current and perhaps modified Emergency Operating Procedurec.
Primary candidates for modificetions are initiation times for water level reduction, boron injection and depressurization. They will develop representative plant parameters for the calculations.
The BWROG informed the staff that ATWS acceptance criteria (Reference 1979 Mattson letter) could be exceeded in the calculations (e.g., tuel temperature limits), and that significant fuel failure is likely. The next meeting to discuss the technical progress in the program is scheduled for August 17. The portion of the meeting requiring the attendance of upper management from NRC (W. Russell and A. Thadani) and BWROG (f. Sears and D. Galley) will be addressed in the BWR0G management meeting to be scheduled about the same time.
NUMARC PRUCUREMENT INITIATIVES The V10 statt was informed by telephone that on June 28, 1990, the NUMARC Board of Directors voted on and agreed to n.ove f orward with the Comprehensive Procurement Initiative.
The initiative includes improvements in licensees' progr6us for performance-based vendor audits, enhanced performance tests and inspections of procured. items, reviewing alternatives f or replacement of obsolete items, sharing vendor audit results tnrough joint audit forums, and increased engineering involvement in the procurement process.
Under the initiative, licensees are to review and assess the program changes needed to implement the comprehensive initiative by July 1,1991 and to tully implement the initiative by July 1, 1992. An initiative on commercial-grade dedication was previously approved for implementation by January 1,1990.

Office of Nuclear Material Safety and Safeguards items of Interest Week Ending July 6,1990 HIGH-LEVEL WAS1E MANAGEMENT Performance Assessment Staft Visit to Sandia National Laboratory (SNL)
On June 25-26, High-Level Waste Management technical staf f visited SNL to discus:;
performance assessment of the potential Yucca Kytntain repository. The NRC staff was given an overview of work being done by SNL in support of DOE. This was followed by discussion of recent performance assessment calculations that were made separately. by the NRC and SNL staff s.
There was also discussion of SNL's development of repository event trees and their approach to analyzing potential impacts of the exploratory shaf t on repository performance.
SAFEGUARDS AND TRANSPURIATION International Large Scale Reprocessing Plant Safeguards (LASCAR) Meeting trom June 18-21, a member of the International Safeguards Branch statt partici-pated 65 a member of the U.S. Government interagency team attending the Third Plenary meeting of LASCAR in Avignon, France. The LASCAR Project, conceived as a result of a Japanese initiative, has the objective of identifying technical features of large scale reprocessing plants that will 1mpact tne application of international sateguards.
Accomplishments of the Third Plenary meeting included:
- 1) a f1nal detailed review of the Working Group 1 report on safeguards consider-ations in spent fuel ponds, 2) review of the progress reports of Working Groups evaluating the main process and storage areas, and 3) review of a proposed joint
. paper on LASCAR by Japan and the International Atomic Energy Agency to be pre-sented at the July 1990 Institute of Nuclear Materials' Management meeting.
Meeting with General Accounting Office (GA0) on Tritium On July'3, 1990, International Safeguards Branch staff met with B. Leavens, GAO, to provide additional information on safeguards requirements for special nuclear material, regulations governing byproduct materials / export activities, and Nu-clear Material Safety and bafeguards (NMSS) licensing and inspection f unctions.
LOW-LEVEL WASTE MANAUtMENT Decommissioning Fathfinder Generating Plant Un June 28, 1990, Region IV staff issued a license amendment to Northern States Power Company for the decommissioning of the reactor building and fuel handling buiIding at the Pathfinder Generating Plant.

Office of Governmental and Public Affairs items of Interest Week Ending July 6,1990 B ate Programs Vogtle Public Officials Meeting Robeit Trojanowski, Region 11 State Liaison Utficer, participated in a meeting with local public officials from around the Vogtle Nuclear Plant on July 2, 1990 in Waynesboro, Georgia. Tne purpose of the meeting was to discuss NRC's program for regulating nuclear power plants and to encourage dialogue with local public officials.

Critisn Utticial Visits Barnwell LLW Disposal Site On July 5,1990, Robert Trojanowski, Region 11 State Liaison Officer, accompanied Mr. Ben Walters, Director, Food Safety Radiation Unit, Ministry of Agriculture, Fisheries and Food, United Kingdom, on a tour of the Barnwell LLW disposal facility and to a meeting with South Carolina regulators on low-level radioactive waste disposal.
International Frograms Strosnider Transfers to NEA Jack Strosnider, Region 1, has accepted a two-year position in the nuclear saf ety division at tht. Nuclear Energy Agency in Paris.
His major responsibility will be for the Committee for Safety of Nuclear Installations

Telephone Problems at Headquarters During the past week, telephones at One White Flint North, Nicholson Lane North /Nicholson Lane South,'and the Nuclear Regulatory Commission's Bethesda locations have been experiencing problems.
The cause of the problems appears to be the conversion of the C&P Montrose Central Office to a more sophisticated digital central office. All problems have been reported to C6F on.a daily basis, and have been brought to the attention of C&P higher management.

Charlotte Guthrie trom Power Computing Compan,y spoke on the topic "bof tware Trail Markers".
She discussed the important milestones in the software life cycle where cocumentation is required in order to assure and verify quality, the monthly Software Quality Assurance Seminar Series will'not meet during the sumer months.

Uttice of Small and Disadvantaged Business Utilization and Civil Rights items of Interest Week Ending July 6,1990 Civil Rights Program A number of agency employees, sponsored by OSDBU/CR, represented NRC by participating in the National Urban League's Black Executive Exchange Program (BEEP) Conference held this year in Atlanta, Georgia, June 20-25, 1990.
The theme was "Welcome to the 90's: A New Decade of Challenges!".
BELP provides the agency with an opportunity for dialogue between corporate and government executives and professionals, and faculty and students at Black colleges and universities.- Discussions centered on how theory, as presented in the classroom, can be put into actual practice in the work place.
Small and Disadvantaged Business utilization Program OSDBU/CR represented the agency at the U.S. Small Business Admin 1stration's Small Business Procurement Opportunities Fair: Expo 90 in Atlanta, Georgia, June 26-27, 1990. The Fair featured a major address by Mr. Wi,lliam Montalto, the Director for the Small Business Committee, U.S. Senate. Approximately 650 exhibitors participated representing both the private and public sector.
Seminars were scheduled over the three day period and included such topics as finencing your business, minority business programs, marketing the Federal government, protests and appeals, and special emphasis programs (women owned business and veterans).

An Order Modifying License _(Ettective immediately) and Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $7,500 was issued on June 26, 1990 to Mississippi X-Ray Service. Inc., Wesson,.
Mississippi. The action was based on violations of requirements for industrial _ radiography that occurred on April. 26, 1990 near Richmond, Virginia. Mississippi X-Ray Service, Inc. is a State of Mississippi licensee that was operating in Virginia under the.Part 150 General License reciprocity. The violations included failure to maintain propor surveil-lance of operations, failure to post radiation or high radiation areas, tai-lure to perform appropriate surveys af ter each exposure, and failure to notify HRC three days prior to operating (in a non-Agreement State.. Ineffective im addition, an Order Modifying the License weekly not.ification to NRC of planneo radiographic operations in non-Agreement States.- (EN 90-063) 2.
A Notice of Violation and Imposition of Civil Penalty in the amount of-
$50,000 was issued on June 27, 1990 to Georgia Power Conipany (Vogtle).
The action was based on the licensee's failure to protect Sateguards Information.
The adjustnient tactors in the Enforcement Policy were considered in determining the amount of the civil penalty, and the base amount was mitigated due to the licensec's identification of the violation and the corrective measures that it implemented following that identifica-tion.- However, the base amount was also escalated due to the licensee's continued poor performance in this area.

An Order Imposing Civil Penelty in the aniount of $50,000 was _ issued on July 2,1990 to Duke Power Coupany (Catawba Nuclear Station, McGuire Nuclear Station, and Oconee Nuclear Station). The action was based on numerous-violations logged at all three sites over a two year period-
' relating to access control, compensatory nicasures, protection of-Safeguards 1nformation, personnel and package search, and training. The licensee responded to the Notice in a letter dated January 31, 1990. After careful consideration of the licensee's response, the staf f has concluded that, with tne exception of two of the examples, the violations occurred as set f orth in the Notice of Violation and Proposed Imposition of Civil Penalty.

July 2,1990 to Deaconess Hospital, Oklahoc4a City, Oklahoma.
The action was based on violations invciving falsification of an NRC-required record of a dose calibrotor constancy test and failure to perform the constancy test on a number of occssions.

Office of Consolidation items of Interest Week Ending July 6,1990 decond Building Status Un July 2, Mr. S) ringer again participated in a working session with the GSA and WFN Partners 11p negotiating teams to complete a lease agreement draft for Two White Flint North (TWFN). Agreement was reached on most oi' the remaining issues with the exception of the rent rate as of the beginning of the lease period. This'is a difficult issue which the GSA negotiating team felt it could not resolve without consulting USA Regional and Central office management. The chief GSA negotietor. indicated that he would consult his management during the week beginning July 9.
Meanwhile, the hearing before the Montgomery County Planning Board on the site plan has been scheduleo for July 12.
However, Albert Abramson, a principal in the WFN Partnership, has stated categorically that if the remaining issues are not resolved and an agreement reached before July 12 he will not go to the hearing and seek approval.
On July 5, members of the Office of Consolidation-(CONS) staff met with GSA, lower, and Dewberry & Davis (D&D) representatives to discuss the process and schedule for interior space planning for TWFN.
On July 6, the Dewberry & Davis interior space planning team leader met with Mr. Springer and Mr. Iselin to lay out a schedule for the interior space planning process for h fN.
Ine schedule assumes a start date for space planning in mid-November 1990, after D&D completes a systems furniture standards review and CONS develops a stacking and blocking plan for TWFN.
The entire interior space planning process is expected to take about 12 months.

Entorcenient Conference With United States Department of Agriculture (USDA)
Inspectors from_ Regions 1,11, IV, and V conducted inspections at twenty USDA f ecilities in 1990.
One of these inspections, conducted March 20-23, 1990, at the W1111em B. Russell Agricultural Research Center in Athens, Georgia, identified a large number of violations. A Confirmation of Action let1.er sent to USDA on March 30, 1990 described corrective actions taken to acidress the problems identified at the Athens tacility.
In addition tc' the findings at the Athens, Georgia, f acility, examples of similar problems were found at other USDA facilities, although not so numerous.
An enforcenent conference has been scheduled with USDA to discuss these findings and the apparent radiation saf ety program weak-nesses which led to the findings. _The enforcement conference, originally scheduled f or July 10, 1990, has been rescheduled for 1:00 p.m., July 11, 1990, so that the Deputy Director of the-Agricultural Research Service can attend.

Seabrook An automatic turbine " setback" from 75% to 40% power occurred at 8:04 a.m., July 2, when operating banks of the main generator transformer cooling fans were shifted during a weekly equipment rotation.
Following the setbbck, a power increase back to 75% was initiated at 1:30 p.m.
At 6:16 p.m., with reactor power at 56%, a controlled power reduction was initiated because of pipe wall thickness measurements taken on July 2 on an elbow in a line from the heater drain tank to the condenser.
Reactor power was reduced to 10% and the main turbine was removed from service at 12:37 a.m., so that the elbow could be isolated.
Preparations for further measurements and repair are in progress. A different section (a reducer) in the same line caused a controlled turbine shutdown over the weekend due to a througn wall leak. The July 2 measurements were a result of the earlier problem.
At 11:5E p.m. on July 2, off site phone communications, including the ENS phone, were degraced as a result of five deliberately cut phone lines at the Seabrook telephone transfer station (off site). The licensee had communications via a microwave communication system (three lines available) and used that system to notify the NRC. All phone communications were restored about 7:30 a.m., July 3.

Pilgrim On Ju ly 2 at 5:11 a.m., with the reactor at 100% power, the "A" recirculation pump tr.otor generator (M-6) set tripped. Reactor power automatically ran back and stabilized at 6b%, as cesigned. The M-G set tripped as a result of a generator lockout; however, no alarm or flogs JULY 6, 199C ENCLOSURE N

- were received prior to the event. About 5:00 a.m., July 3, an unusual event was declared,-as-required, when the M-G set problem could not be corrected. The licensee is taking. the plant to cold shutdown to correct this problem and perform other maintenance.
An about seven-day outage is anticipated.

International Visitor On July 5, Ben Walters, a representative of the United Kingdom, and the Director, State Programs, met with State officials in Columbia, South Carolina to discuss matters of mutual interest.
In conjunction with this meeting, Mr. Walters also toured the Barnwell Low Level Weste disposal facility.

Meeting with Local Otticials On July 2, members of Regional management and the Vogtle resident inspectors met with public officials trom the City of Waynesboro and Burke County, Georgia.
Jtems discussed included the NRC's mission, Region ll's operating objectives, the Resident Inspectors' program, and an update on recent operational activities at the site.

Kewaunee Nuclear Power Station On Tuesday, July 3,1990, Mr. H. J. Miller, Director, Division of Reactor Safety and members of his staf f meet witn representatives from Wisconsin Public Service Company (Kewaunee Nuclear Power Station) to discuss the-results of the licensee's self-initiated Safety System Functional Inspections for the Emergency DC Power System and Service Water System.
In addition, the licensee presented an update regarding its proposed actions to address Generic Letter 89-13.

Duane Arnold Nuclear Power Station On June 24 through 29, 1990, a requalification examination was conducted at the Duane Arnold Nuclear Power Station. The NRC failed four individuals with the f acility talling eight individuals and both the NRC and the f acility failing three of four crews. The crew failures made the program unsatisf actory. A meeting was held July 5, 1990, with lowa Electric Light and Power Company management to discuss the course of action that will neec to be taken to correct this problem. The unit was shut down for refucling oon June 27, 1990, and is scheduled for return to service the first week of September.

Unintentional Overexposure of Intant's Thyroid (Update)
Region it staff, HMSS and an NRC Medical Consultant have completed.the onbite inspection and are continuing to evaluate an iticident at the Tripler Army Medical Center in Honolulu, Hawaii.' A nursing mother was given a 4.89 millicurie dose of iodine 131 on June 19,1996 and a substantial fraction of that cose was passed to the mother's nursing infant resulting in a thyroid gland ablation doss to the intant.
i Extensive dose essessment by the Oak Ridge National Laboratory utilizing-data from whole body and thyroid scans and analysis of two breast milk samples incicate a dose to the intent's whole body of 77 rem and 30,000 rem to the thyroid gland.
