Docket No. 50-263 License No. DPR-22 Failure of LPCI Injection Valve to Open A condition occurred at the Monticello Nuclear Generating Plant which we are reporting to your office in accordance with Section 6.6.C. of the Technical Specifications, of Provisional Operating License DPR-22.

On April 5, 1973, during routine surveillance testing, the No. 11 LPCI inboard injection valve, FD-2014, could not be opened using the control room handswitch. Dirty auxiliary contacts were found to have prevented energizing the opening contactor coil circuit. The contacts were cleaned and the valve was demonstrated to operate properly.  !
