APPEEDIX A NOTICE OF VIOLATION Westinghouse Electric Corporation Docket No. 70-1151 Nuclear Fuel Division License No. SNM-1107 As a result of the inspection conducted on June 2-3, 1982, and in accordance with the NRC Enforcement policy, 47 FR 9987 (March 9,1982), the following violation was identified.

License Condition 19 requires the licensee to operate and maintain the nuclear plant facility in accordance with the statements, representations and conditions contained in Sections 2, 3, and 4 of the licensee's applica-tion dated December 30, 1976.

Section 3 of the licensee's application states that the activity will be operated in accordance with general operating philosophy that is employed in Westinghouse plants and facilities which includes safety and health protection. Basic policies and procedures are established by plant management.

Licensee's Procedure No. SY-507, Cutting and Welding Policy, provides the safety requirements which must be followed for all cutting and welding operations.

Contrary to the above, the requirements of Procedure SY-507 were not being followed on June 2, 1982, for the oxyacetylene gas cutting and arc welding operations being conducted in the chemical manufacturing area in that:

1.

Welding permits were not provided for each operation or area in which welding and cutting operations were being conducted; 2.

Combustible materials within 35 feet of the welding and cutting operations were not removed or properly covered; 3.

Fire watch was not provided for welding and cutting operations; and 4.

Fire extinguishers were not available at each welding and cutting operation.

This is a Severity Level V Violation (Supplement VI).

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged viola-tion; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken,to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
