WESTINGHOUSE RESPONSE TO tEC NOTICE OF VIOLATION Concerning the requirements of Condition 19 and Section 3 of Columbia Plant License SNM-1107 and Plant Procedure SY-507, " Cutting and Welding Policy," the following comments are provided:
1.
The violation is correct as stated in your Notice of Violation.
2.
Due to an oversight in training, some of the responsible personnel, especially non-Westinghouse construction personnel, had not received adequate instructions relative to the proper safety precautions that shall be taken during cutting and welding operations.
3.
The cutting and welding operators that were observed during the subject compliance inspection were non-Westinghouse construction personnel.
The details of Plant Procedure SY-507 have since been reviewed with, and acknowledged by, the non-Westinghouse cutting and welding operators.
In addition, the following corrective actions have been taken to enhance personnel safety and health protection during these operations:
Cutting / Welding Permits are now issued and posted in accordance wth Plant Procedure SY-507.
The cutting and welding operators have been cautioned in a workplace meeting about welding over or near combustible materials, and instructed to use proper shielding to protect conoustibles that cannot be removed from the welding area.
Fire watch assignments are now made at the time Cutting / Welding Permits are issued.
Fire extinguishers are being made available for all cutting and welding operations. Training classes on the proper use and handling of fire extinguishers are provided by the Safety Section, and training for the cutting and welding operators is scheduled to be completed by August 10, 1982.
4.
A review' of Plant Procedure SY-507 will be conducted for Westinghouse personnel who perform cutting and welding operations by August 10, 1982.
Periodic audits of the cutting and welding procedure and operations will be conducted on a continuing basis and should preclude any recurrences.
5.
Full conpliance was achieved, except for the described training sessions, on June 4, 1982. The additional training sessions are scheduled for canpletion by August 10, 1982.
