Dear Congressman Fascell:
D. Eisenhut L. Underwood J. Heltemes H. Thompson This letter is in response to your letter of June 18, 1982, which made reference to the Turkey Point Plant Unit Hos. 3 and 4 steam generator problem.
As a matter of background infomation all three steam generators of the Turkey Point Plant Unit No. 3 have already had the steam generator tube section replaced and the unit has been operating since April 1982. Turkey Point Unit No. 4 is scheduled for a similar repair operation to begin in October 1982. Both units have had an essentially leak-free operation for over three years (no leaks clearly above the threshold of detection).
During the weekend of June 5/6 Unit 4 shutdown because of several steam leaks from inside the shield wall at the hand hole cover and at valves on the steam generator secondary side. The increase in temperature and humidity led to a licensee decision to go to cold shutdown and repair the leaks.
With the steam generators drained the licensee also examined the steam generator primary side. Ilydrostatic pressure was applied to the secondary side with the primary side open so that the tubc. sheet was visible. Three tubes in steam generator C which had been preventively plugged in a previous inspection were wet. This is characteristic of what is temed a " weeping" plug and no action was taken. A " weeping" plug is simply a tube that has been plugged, has no measurable leakage and yet shows wetness in a hydro-static test.
In steam generator C the same hydrostatic test revealed a tube which dripped water at a rate which was below the threshold of detection. However this tube and the four imediately surrounding it were preventively plugged. Steam Generator A showed no wetness at the tube sheet.
The measurement of the rate of primary to secondary leakage prior to shutdown showed a fairly constant 0.6 to 1.0 gallons per hour. This has been typical for " weeping" plug (s) at the Turkey Point Plant in the past and is about the threshold level for detecting leaks for that plant. The leak rate limit at which the facility is required to shutdown for steam generator tube inspection is 18 gallons per hour.

In response to question number 1. I enclose a copy of Amendment No. 80 which was issued for Turkey Point Unit No. 4 to pemit operation until October 1982.
Included is the Safety Evaluation Report which is the basis for the issuance of the amendment. Briefly, the conservative plugging program, which has proved successful over the past four years, the leak-free operation of both units, the operation of other similarly degraded steam generators, the extensive inspection of both denting and wastage and finally, special conditions regarding steam generator tube leakage were added to the already existing special license conditions.
In response to question number 2, there are several special actions now under consideration by the staff, based on recent experience which includes the Ginna information.
Regarding the Turkey Point Plant Unit Hos. 3 and 4, those units already have certain of the additional requirements being considered and have been operating under special surveillance requirements. They also presently have loose parts monitoring systems, which are currently being updated.

These actions have not been issued as yet but the staff has under consideration such specific requirements as television inspection, of the secondary side of the steam generator, loose parts monitoring systems, more stringent surveillance requirements for the steam generator tbbes, etc.
Broad areas of consid-eration are steam generator integrity, plant systems response, human factors, radiological consequences and organizational response.
