Dear Sir:

The NRC was notified of a potentially reportable deficiency in the referenced telephone conversation. At that time, it was estimated that a determination of reportability would be made within thirty (30) days.

Due to the extensive investigation and evaluation required, an Interim Report is attached.

It is now expected that this information will be finalized by August 23, 1982, at which time a complete report will be submitted.

INTERIM REPORT - DER 82-33 POTENTIALLY REPORTABLE DEFICIENCY ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT 1 I.

POTENTIAL PROBLEM A three-inch Swing Check Valve, Equipment Tag No. IPIAEV-073 in the Instrument Air System, supplied by Anchor-Darling, contains a missed factory operation.

Set Screw (PC-14) was not tack welded in place per ADV drawing requirement. This condition allows valve leakage past seat.

II.

APPROACH TO AND STATUS OF PROPOSED RESOLUTION Repair valve in accordance with Procedure No. 31-MT-9ZZ01; when adjust-ments are. complete, stake set screw in three places (each) as discussed with Anchor-Darling.

The Tech Manual drawing will be changed to delete the

" tack weld" statement when the " Facility Modification Control" procedure 73AC-0ZZ12 is implemented.

III.

PROJECTED COMPLETION'0F CORRECTIVE ACTION AND SUBMITTAL OF THE FINAL REPORT Evaluation of this condition and submittal of the Final Report is Forecast to be completed by August 23, 1982.
