This refers to the inspection conducted by Mr. D. P. Loveless, and the
i
inspectors identified in the attached report, of this office on September 26
!

through November 6, 1993.
The inspection included a review of activities
,
l
authorized for your South Texas Project, Units 1 and 2, facility. At the
i
!
conclusion of the inspection, the findings were discussed with those members
l
of your staff identified in the enclosed report.
i
,
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
)
and representative records, inspection of installed equipment and system
i
alignments, interviews with personnel, and observations of activities in
l
progress. The purpose of the inspection was to determine whether activities
l
authorized by the license were conducted safely and in accordance with NRC
l
requirements.
Based on the results of this inspection, certain activities appeared to be in
violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice).
The violation is of concern because your lack of control
-)
over contract persom el contributed to the potential compromise of the

integrity of a safety-related system.
Based on this concern, your response
l
should include a discussion of any necessary revisions to the procedural
controls that govern contractor support and requirements for Houston Lighting
& Power Company employee oversight of contractor activities.
Additionally, one unresolved item was identified on fuse configuration
i
control. A future NRC inspection will evaluate your investigation and
i
l
corrective actions for four issues involving improperly sized fuses installed
!

in the plant. At that time, a determination will be made whether NRC
l
requirements were violated.
l
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.

this letter and its enclosures will be placed in the NRC Public Document Room.
The response directed by this letter and enclosed Notice is not subject to the
clearance procedures of the Office of Management and Budget, as required by
the Paperwork Reduction Act of 1980, PL 96-511.
