- Catawba Nuclear Station, Units 1 and 2 Docket Nos. 50-413.and 50-414 NRC Inspection Report No. 50-413, 414/90-24 Reply to Violation 50-414/90-24-02 Request for Extension Gentlemen:
Dusing the Catawba Unit 2E003 refueling outage, the SSF was in degrade when Mode 3 was entered. Operations personnel failed to follow Operating Procedure OP/2/A/6100/01, Controlling Procedure for Unit Startup. The Control Room Senior Reactor Operator (CRSRO) initialed steps-in the procedure verifying the SSF as being operable and that any item preventing entry into Mode 3 had been cleared, when in fact neither was the case. Thus the degraded condition of the SSF was not identified. You have identified t.his as 'another exampic of f ailure to follow procedure similar to that
'identifLed in NRC. Inspection Report 90-24 and that we are to include this example in our response to Violation 414/90-24-02.
At.this time, we request an extension to November'12, 1990 for submittal of this response. This extension period would provide us with the time necessary for additional investigation into this event to determine root causes and necessary corrective actions to be taken to preclude recurrence-of this incident.
