L Although you ~ investigated this matter earlier, we are concerned that you failed to identify the root cause.of the p,roblem.- It appears that management involvement in the investigation was lacking such that employee / contractor y

l cooperation was not elicited.

It also appears that management did not respond L

to the failure on the.part of employees / contractors to cooperate with an investigation. We request that fou review the adequacy of your initial l

investigation and address the-lack of cooperation on the part of employees.

' We request.that the results of-your review and disposition of this matter be submitted to Region III-within 30 days of the date of'this letter. We request that your responsc contain no personal privacy, proprietary, or safeguards information'so.it' can be-released to the public and placed in the NRC Public Document Room.

If necessary, such information shall be contained in a separate attachment which will be withheld frc.m public disclosure and the affidavit required by:10'CFR 2.790(b) must accompany your response.
