As a result of the loss of feedwater evett of April 24, 1982 at the Vemont Yankee
l
Huclear Power Station, we initiated two separate activities to obtain infomation
regarding the event and the circumstances associated with it. By letter dated
,
May 12, 1982, you were informed of the results of a special NRC inspection related
primarily to the technical aspects of the event. This letter refers to an investi-
gation conducted by Mr. R. Keith Christopher of the NRC Region I Office on June

6-10, 1982 at the Vermont Yankee facility and elsewhere.
Areas examined during this investigation are described in the enclosed NRC Investi-
,
gation Report. Within these areas, the investigation consisted of interviews with
personnel and examination of selected procedures. The sworn statements obtained
from the interviews and transcripts of the two phone reports on April 24, 1982 to
the NRC Headquarters Duty Officer are included.

The purpose of the investigation was to detemine if there was willfullness or
careless disregard in withholding infomation from the NRC while making required
notifications to the NRC duty officer and to examine the reason (s) for not making

notifications to appropriate offsite agencies and officials in view of the nature

of this event. ' Based on our review of the facts, infomation and sworn statements
obtained during this investigation, we do not find evidence of wi11 fullness or of
careless disregard for Commission regulations or requirements.

developed as a result of this investigation is being considered in our continuing
evaluation of the event.
In this regard, we are conducting an extensive evaluation of this event and the
- circumstances associated with it to better understand the significance of the

underlying reasons for the problems that were encountered. Specifically, the
'prvbHas involved with analysis, classification and notification do not appear to

be isolated issues. As we have previously stated, in this particular situation'
the event did not warrant escalated emergency actions; however, this does not
lessen our concern for the adequacy of the measures that are being taken to provide
information in a reasonably prompt and reliable manner to appropriate offsite
