SUBJECT: NRC INSPECTION REPORT 50-482/93-27 (NOTICE OF VIOLATION)

This refers to the inspection conducted by Messrs. G. A. Pick and

J. F. Ringwald of this office on September 12 through October 23, 1993.

The

inspection included a review of activities authorized for your Wolf Creek

,

Generating Station facility. At the conclusion of the inspection, the

findings were discussed with you and those members of your staff identified in

the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observation of

activities in progress. The purpose of the inspection was to determine

whether activities authorized by the license were conducted safely and in

accordance with NRC requirements.

Based on the results of this inspection, certain licensed activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice).

Violation A pertains to a failure of operations personnel

to properly implement the requirements of a clearance order.

Evan though your

staff identified the deficiency and performed a thorough review of the issue,

this example was cited because clearance order implementation problems

,

continue to be identified (refer to NRC Inspection Reports 50-482/93-03

and 93-19).

Violation 8 pertains to a failure of personnel to immobilize equipment as

!

required by the temporary equipment control program, which indicated that

i

j

personnel did not fully understand the program requirements. This violation

'

is of concern because equipment that is not immobilized could be a hazard to

i

personnel and equipment during a seismic event.

)

Violation C is of concern because personnel failed to properly follow scaffold

program controls in two NRC identified examples. The second example occurred

before personnel could implement corrective actions in response to the first

i

deficiency.

Your response to the second example was more comprehensive than

your response to the first example and suggests that you will substantially

enhance your scaffolding program to prevent occurrence of this problem.

Violation D resulted from personnel beginning work without the shift

supervisor's permission. This violation is of concern because craft personnel

are expected to adhere to station procedures and to keep the shift supervisor

fully informed of work activities.

Violation E resulted from the identification by NRC of inadequate corrective

actions as described in a licensee event report. This violation is of concern

because the corrective actions implemented were not thorough and may not have

been sufficient to prevent recurrence.

You are required to respond to this letter and should follow the instructions

spari4'ind in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions you plan to p avent recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

this letter and its enclosures will be placed in the NRC Public Document Room.

The responses directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
