This refers to the safety inspection conducted by Mr. P. Swetland and others of this office on

l

August 18,1993 through September 28,1993, at Millstone Station in Waterford,

Connecticut. The preliminary findings were discussed with Mr. S. Scace, the Unit

Directors, and others of your staff at the conclusion of the inspection. Areas examined

during the inspection are described in the enclosed report. Within these areas, the inspection

focused on issues important to public health and safety, and consisted of performance

observations of ongoing activities, independent verification of safety system status and design

configuration, interviews with personnel, and review of quality records.

The inspectors identified that certain requirements of the Unit 2 license were not

implemented during the inspection period. These included failure to follow procedure steps

for the calibration of radiation monitoring instruments, and failure to conduct inservice

inspections of auxiliary feedwater system pipe supports. Two violations of NRC

requirements concerning these problems are cited in the enclosed Notice of Violation. Your

attention is directed to the fact that these self-identified violations have been cited because of

the recurrent nature of procedure adherence problems in the Unit 2 instrumentation

organization, and the prolonged period during which inservice inspection was not performed

on the auxiliary feedv/ater system pipe supports despite several reviews of potential generic

weaknesses on these supports. In your response to the Notice of Violation enclosed with this

letter, please provide your evaluation of the need for remedial action regarding your self-

assessment and corrective action programs.

You are required to respond to the enclosed Notice of Violation and should follow the

l

instructions in the enclosed Notice when preparing your response. In accordance with

l

10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and the enclosed

Notice wi!! be placed in the NRC Public Document Room. The responses directed therein

are not subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, Public Law 96.511.

During this inspection we also evaluated your self-identification and correction of problems

related to a non-seismically mounted service water branch line at Unit 1 and failure to

conduct a containment closure evaluation for steam generator manways at Unit 3. We

determined that your corrective actions for these issues were acceptable and the specific

,

'

problems had minimal safety consequences. Therefore, in accordance with Section Vll.B of

the Enforcement Policy, enforcement discretion was exercised and violations were not cited

for these cases.
