On July 13, 1990, the NRC issued CorHrmatory Action Letter (CAL-Rill-90-013)

I to document your proposed,orrectiva actions to the unsatisfactory licensed

- operator requalification training program. The program's unsatisfactory-status was based on the number of ino'vidual.: who failed the requalification examination conducted on June 25-29, l.T90 (E1 amination Report No. 50-331/0L-90-02).

Paragraphs A and B of the CAL requi*ed accelerated' remedial training

and reexamination of those individuals who ta11e.1 the requalification exam.

Additionally, an Operational-Evaluation was to be conducted on the three i

Loperating crews which were not-examined in June prior to taking the reactor above the-refuel mode or the cold shutdown condition pursuant to the modified CAL dated. August 8,-1990 (revised Item B and additional Item J).

During the week _of August 13-17, 1990, Mr. H. Peterson led an examination team 4

'which conducted the Operational Evaluation and the reexamination.. The H

Operational. Evaluation included dynamic simulator examination of the three.

' heretofore unexamined operating crews. LThe requalification reexamination examined the three crews and eight individuals who-had failed the original

- l requalification exam._ All crews and individuals were judged as ' satisfactory with the exception of one individual's failure in the Operational Evaluation.

~ 1 Your. attention is ~ invited to the Details section of the examination report (Examination Report No. 50-331/0L-90-03), which delineates the scope of these evaluations. At the conclusion of the examination, Mr. Peterson presented evaluation results to Mr. D. Mineck and others of your staff on August 17, 1990..

We have reviewed DAEC internal letter 005175 from the Plant Superintendent-I Nuclear to all " Inactive NRC License Holders DAEC" dated July 18, 1990 which restricted their ab_ility to re-activate their license until enhanced training

and. evaluation in accordance with CAL has been completed.

The enhanced training is understood to reflect the additional knowledge and ability-P-

_ deficiencies: identified in the June requalification exam, the August Operational Evaluation, and your formal root _ cause evaluation discussed in

. Item F of-the CAL.. Additionally, it is understood that licensed staff enhanced training commenced during the week of October 8, 1990.

Power Company Based on our review of your documentation and the results of the Operational Evaluation-and reexamination, we consider CAL Items A, B (modified), C, D, E, G, I, and the added Item J to be complete and closed.

Copies of documentation associated with the subsequent enhanced training for the next requalification 4

training cycle, in accordance with Items C and G, need not be submitted to the Region Ill office, but should be available for review on site.

We received your latest CAL response on September 6, 1990 (DAEC letter NG 2088, dated August 25,1990), concerning the root cause and corrective action plan regarding the operator requalification training program.

This analysis adequately meets the requirements of CAL item F.

It is understood that after the Operational: Evaluation, the evaluation findings validated the results of the root'cause analysis and, therefore, an update of the corrective action plan in accordance with item H is not required.

As a result of the NRC review of the root cause' analysis, the satisfactory Operational. Evaluation and reexamination, NRC observations of continued sound evaluations made by the facility evaluators, and the much improved simulator i

scenarios, the NRC recognizes.that.the facility training program is adequate i

to remediate, reexamine, and return to licensed duties subsequent requalification failures without direct NRC participation pursuant to

-10CFR55l.59.(a)(2)ili.

Inasmuch as lowa Electric Light and Power Company has satisfactorily addressed all items in CAL-Rill-90-013, the NRC considers the

-CAL,to be closed.

It should-be noted that while the CAL is considered closed and the facility

'may resume normal requalification program activities, the training program is

- still documented as unsatisfactory until another complete requalification examination is administered and evaluated by the NRC, in accordance with

'NUREG-1021, Operator Licensina Examiner Standards..
