SUBJECT:

NOTICE OF VIOLATION (NRC INSPECTION REPORTS NO. 50-254/93024(DRS);

NO. 50-265/93024(DRS))

This refers to the team inspection conducted by Mr. G. M. Hausman and others

of this office on July 28-30, August 2-4, and August 9-20, 1993. The

inspection included a review of activities authorized for your Quad Cities

Nuclear Power Station, Units 1 and 2.

At the conclusion of the inspection,

the findings were discussed with those members of your staff identified in the

enclosed report.

Areas examined during the inspection are identified in the report.

The

purpose of the inspection was to determine whether activities authorized by

the license were conducted safelv and in accordance with NRC requirements.

The inspection included a revie.

' your activities associated with an unusual

event declared on July 27,1993, dt.2 to loss of offsite power.

The inspection

also included an assessment of the quality and effectiveness of electrical

maintenance activities associated with high, medium and low voltage breakers.

Within these areas, the inspection consisted of selective examinations of

procedures and representative records, interviews with personnel, and

observations of activities in progress.

Although overall breaker maintenance was considered adequate, post-maintenance

'

testing on bus 22 breakers prior to the unusual event was inadequate.

Failure

by plant personnel to recognize the RAT to UAT slow transfer existed per plant

design was seen as a weakness in personnel training.

In addition, substation

construction (SSC) documentation related to the identification of preventive,_

predictive and corrective maintenance activities on oil circuit breakers and

'

,

associated 345kV switchyard equipment was also seen as a weakness. The lack

of documented activities on nonsafety-related equipment that can affect plant

operations is a concern to the NRC. Therefore, in addition to the response to

the enclosed Notice, please provide a written response on the actions you plan

to take with regards to improving SSC documentation.

Based on the results of this inspection, certain- of your activities appeared

to be in violation of HRC requirements, as specified in the enclosed Notice of

'

Violation (Notice). The violations are of concern because of the potential

safety impact inadequate electrical breaker maintenance could have on plant

operations.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional-

actions you plan to prevent recurrence. After reviewing your response to this

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether further NRC enforcement action is

-necessary to ensure compliance with NRC regulatory requirements.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter, its enclosures, and your response to this letter will be placed

'

in the NRC Public Document Room.

The responses directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
