Appendix A NOTICE OF VIOLATION Censumers Power Company Docket No. 50-330 Based on the results of an NRC inspection conducted on August 29-31, 1978, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below. This item is an infraction.
10 CFR 50, Appendix B, Criterion V, states, in part, that " Activities affecting quality shall be prescribed by documented instructions, procedures,.... and shall be accomplished in accordance with these instructions, procedures.
.." Consumers Power Company Quality Assurance Program Policy No. 5, Rev. 5, Paragraph 2.0.k referenced ANSI N45.2, Criterion 6.
Criterion 6, states, in part,

that, " Activities affecting quality shall be prescribed by documented instructions, procedures,
. and shall be accomplished in accord-ance with these instructions, procedures.
Bechtel Power Corporation Welding procedure No. P8-AT-Ag, Rev. 1, requires that backing gas flow rate be 10-15 CFH for welding first and second passes.
Contrary to the above, on August 31, 1978, the inspector determined the backing gas flow rate used by the welder when welding the root pass of weld No. 38 on drawing No. M611, sheet 5, was 45 CFH.
