NOTICE OF VIOLATION Southern Nuclear Operating Company, Inc.
Docket Nos. 50-348, 364 Farley 1 and 2 License Nos. NPF-2, NFP-8 During an NRC inspection T nducted August 16 through September 3, 1993, violations of NRC requirements were identified.
In accordance with the
" General Statement of Policy and Procedure for NRC Enforcement Actions,"
10 CFR Part 2, Appendix C, the violations are listed below:

10 CFR 50, Appendix B, Criterion XVI, " Corrective Actions," requires, in part, that measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment and nonconformances are promptly identified and corrected.
In the case of significant conditions adverse to quality, the measures established shall assure that the cause of the condition is determined and corrective action taken to preclude repetition.
The identification of the significant condition adverse to quality, the cause of the condition, and the corrective action taken shall be documented and reported to appropriate levels of management.
Contrary to the above, as of September 3, 1993, measures had not been established assuring that the cause of conditions adverse to quality was determined and corrective action taken to preclude repetition in that the following instrumentation:
Q1P16FT3014BA, Containment Cooler Service Water Outlet Flow Loop on 9/1/93 QlP16FY3014AB, Containment Cooler Service Water Outlet Flow Loop on 3/11/92 Q2P16FT3014AA, Containment Cooler Service Water Outlet Flow Loop on 3/10/92 Q1P16FY3014BB, Containment Cooler Service Water Outlet Flow Loop on 4/10/91 Q2P16FT3014BA, Containment Cooler Service Water Inlet Flow Loop on 3/11/91 was found out of prescribed tolerances without establishing or documenting the cause of the condition nor taking corrective action to preclude repetition.
This is a Severity Level IV violation. (Supplement I.D.3) 2.
Technical Specification 4.0.5.a requires that in-service inspection of ASME Code Class 1, 2, and 3 components and in-service testing of ASME Code Class 1, 2, and 3 pumps and valves shall be performed in accordance with Section XI of the ASME Boiler and Pressure Vessel Code and applicable Addenda as required by 10 CFR 50, Section 50.55a(g), except where specific written relief has been granted by the Commission pursuant to 10 CFR 50, Section 50.55a(g)(6)(i).

valves from the in-service test program.
This is a Severity Level IV Violation (Supplement I.D.3) 3.
10 CFR 50, Appendix B, Criterion V, " Instructions, Procedures, and Drawings," requires that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances, and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished.
Technical Specification 6.8.1.c. requires that written procedures for surveillance and test activities of safety related equipment be established, implemented and maintained.
Technical Specification 4.7.4.c. requires that buried service water piping is verified leak tight by a visual inspection of the ground area every 18 months.
Contrary to the above, on September 1,1993, surveillance procedure, i
FNP-0-STP-24.6, " Service Water Buried Pipe Inspection," did not adequately direct implementation of buried service water piping leak tight visual inspections in that the procedure did not require the normally isolated i
emergency recirculation piping be pressurized and the indicated landmarks could not be used to locate substantial portions of the buried piping.
This is a Severity Level IV Violation (Supplement I.D.3)
Pursuant to the provisions of 10 CFR 2.201, Southern Nuclear Operating Company, Inc. is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission for item 1, ATTN: Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, a copy to the NRC Resident Inspector, within 30 days of the date of the letter transmitting this Notice of Violation. This reply should be clearly marked as a " Reply to a Notice of Violation" and should include:
(1) the reason for the violation, or if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

NPF-2, NFP-8 If an adequate reply is not received in the time specified in this Notice, an order or demand for information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
Corrective actions for the violations addressed in items two and three of this Notice have been completed and the actions reviewed by NRC representatives. Therefore, no reply to these items of the Notice.is required.
